摘  要:[目的]探讨基于CT图像使用Mimics软件量化颈椎后纵韧带骨化(OPLL)容积方法的可行性,并将测量结果与传统X线测量法进行比较。[方法]选取于本院2012年2月~2015年12月在门诊接受非手术治疗的34例颈椎OPLL患者,所有患者均至少先后两次同时接受颈椎侧位X线片和CT平扫检查。由3名高年资骨科专科护师在1名脊柱外科医师的指导下分别于两次检查后使用X线法和Mimics软件法对骨化物的容积进行测量,每次测量均重复1次,对测量结果进行可信度和可重复性分析。[结果]X线片法组中共有5例患者因摄片质量问题而导致测量失败,Mimics软件法组均顺利完成测量。两次测量平均间隔14.3个月。两种测量方法均提示OPLL发生进展,X线法测得的骨化物纵向长度平均进展率为4.52%,Mimics?软件法组骨化物容积的平均进展率为5.82%。X线法的可重复性为较好~极佳水平(ICC为0.810~0.922),可信度均为较好水平(ICC为0.786和0.824);Mimics软件法的可重复性和信度均为极佳水平(ICC≥0.9)。[结论]本研究结果表明,基于CT图像,使用Mimics软件能够更为精确地测量OPLL的容积。[Objective] To investigate the feasibility of quantitatively measuring the volume of cervical ossification of poste- rior longitudinal ligament (OPLL) by Mimics software based on CT-scan images, and compare the results measured by tradition- al X-rays images. [Methods] Thirty-four patients with cervical OPLL receiving conservative treatment in the out-patient de- partment from February 2012 to December 2015 were included in this study. All the patients were taken cervical lateral X-rays and CT-scan simultaneously at two different times. The volume of OPLL was respectively calculated by 3 senior nurses in coop- eration with 1 spine surgeon using the X-ray method and Mimics method. The measurement was repeated once at each time of radiologic examinations. Both intraobserver and interobserver agreements were accessed by calculating the intraclass correlation coefficient (ICC). [Results] Five patients in the X-ray group failed the measurement due to the poor imaging quality, while all the patients in the Mimics group were successfully evaluated with a mean interval time of 14.3 months between two tests. The tests results indicated that all the patients had a progression of OPLL, with a mean progression rate of 4.52% in the X-rays group, whereas 5.82% in the Mimics group. The intraobserver agreement varied from 0.810 to 0.922 ICC, while the interobserv- er agreement from 0.786, to 0.824 ICC in the X-ray group. By contrast, both the intraobserver and interobserver agreements proved ICC≥0.9 in the Mimics group. [Conclusion] The study reveals that measurement of the volume of OPLL using the Mim- ics software is a feasible and more precise method.
