金天格胶囊联合辛伐他汀对骨质疏松性Ⅰ度胸腰椎压缩性骨折疗效的临床观察

摘　　要：目的探讨金天格胶囊联合辛伐他汀治疗老年骨质疏松I度胸腰椎压缩性骨折的疗效及对患者骨密度、骨代谢的影响。方法把128例老年骨质疏松I度胸腰椎压缩性骨折患者随机分为治疗组和对照组。两组患者均进行保守治疗及金天格胶囊干预,治疗组在对照组治疗的基础上给予辛伐他汀治疗。分别在治疗前及治疗后1个月、3个月及6个月评定两组患者视觉模拟评分(visual analogue score,VAS)及Oswestry功能障碍指数(oswestry disability index,ODI)评分;治疗前后6个月检测患者骨密度及骨特异性碱性磷酸酶(bone specific alkaline phosphatase,BAP)和I型前胶原肽(type I Pro collagen peptide,CTX);观察治疗组药品不良反应发生情况。结果治疗1个月、3个月及6个月后两组患者VAS与ODI评分均较治疗前明显改善(P<0.05),且治疗组患者评分明显低于对照组(P<0.05)。治疗后6个月,治疗组患者骨密度较治疗前显著改善(P<0.05),血清BAP和CTX较治疗前明显下降(P<0.05),且均明显优于对照组(P<0.05)。两组药品不良反应发生率比较,差异无统计学意义(P>0.05)。结论金天格胶囊联合辛伐他汀对老年骨质疏松I度胸腰椎压缩性骨折疗效较好,能增加骨密度,降低血清BAP和CTX水平,且安全性佳。Objective To explore the effects of Jin Tiange Capsule and Simvastatin combined with conservative treatment on thoracolumbar vertebral compression fractures,bone mineral density and bone metabolism in elderly osteoporosis patients. Methods128 cases of elderly patients with osteoporotic grade I thoracolumbar vertebral compression fractures were divided into treatment group and control group at random. The patients in the two groups were given conservative treatment,while the patients in treatment group were additionally given Simvastatin and Jintiange capsule. The visual analogue scale（ VAS） and Oswestry disability index score（ ODI） were assessed before treatment and after 1,3 and 6 months the treatment,as well as bone density,bone specific alkaline phosphatase（ BSAP） and type I Pro collagen peptide（ CTX） before and after 6 months of treatment. The effect on bone density and bone metabolism in patients of the two groups were compared,and the adverse drug reactions were observed. Results The VAS and ODI scores of patients in the two groups at 1,3,6 months after treatment were all improved compared with before treatment（ P〈0. 05）,and the scores of patients in the treatment group were much lower than those in the control group（ P〈0. 05）. The serum levels of BSAP and CTX increased significantly compared with pre-treatment（ P〈0. 05）,and all index were better than the control group（ P〈0. 05）. While comparing the incidence of adverse drug reaction in two groups,the differences were not statistically significant（ P〈0. 05）. Conclusion The application of Jin Tiange Capsule and Simvastatin combined with conservative treatment had favorable curative effect on osteoporotic Grade I thoracolumbar vertebral compression fractures in elderlypatients,could improve bone density and reduce the serum levels of BSAP and CTX,and had good safety.
