摘 要:目的应用二维斑点追踪成像(2D-STI)技术测量非对称性肥厚型心肌病(HCM)患者早期左室纵向收缩功能,并分析其相关影响因素。方法连续纳入非对称性HCM患者40例,包括非梗阻性肥厚型心肌病(NHCM)患者31例、梗阻性肥厚型心肌病(OHCM)患者9例,并收集同期健康体检者20例作为对照组。应用2D-STI技术测量左室收缩期整体纵向应变(GLS),并与部分超声参数进行相关性分析。结果与对照组比较,HCM组IVSd、LVEF、LVOT-PG以及LVMI明显增加(P<0.05),左室GLS减低(P<0.05)。GLS与LVEF等参数无相关性(P>0.05),与IVSd、LVOT-PG以及LVMI存在线性正性相关(r分别为0.479、0.316、0.374,P<0.05)。结论 HCM患者早期已出现整体纵向收缩功能减低,多种常规超声参数均与该变化有相关性。Objective To investigate the left ventricular longitudinal systolic function in patients with asymmetric HCM by 2 - D speckle tracking imaging (2D - STI) and analyze the related factors. Methods 40 HCM patients including 31 NHCM patients, 9 OHCM and 20 healthy subjects as control group were collected . 2D - STI technology was applicated to measure left ventricular systolic global longitudinal strain value( GLS), and the correlation of GLS with the ultrasound parameters was analyzed. Results Compared with normal control group, IVSd, LVEF, LVOT - PG and LVMI in HCM group increased significantly ( P 〈 0.05 ) ; Left ventricular GLS re- duced ( P 〈 0.05 ). GLS was not correlated with LVEF ( P 〉 0.05 ). But there was a linear positive correlation with IVSd, LVOT - PGand LVMI (r = 0. 479 ,0. 316 ,0. 374 , P 〈 0.05 ). Conclusion The global longitudinal systolic function has been reduced in early stage of HCM, and some conventional ultrasound parameters are related to this change.
