西地那非治疗老年慢性心力衰竭并发肺动脉高压的效果

摘　　要：目的观察西地那非治疗老年慢性心力衰竭(CHF)并发肺动脉高压(PH)的效果。方法将86例老年CHF并PH患者随机分为两组:对照组和试药组(每组43例)。对照组采用常规治疗方法,试药组在此基础上使用西地那非治疗,比较两组患者的治疗效果、左心功能、肺动脉收缩压(PASP)、超敏C反应蛋白(hs-CRP)及氨基末端脑钠尿肽前体(NT-pro-BNP)水平。结果试药组治疗后左室射血分数(LVEF)(42±11)%、左室短轴缩短率(LVFS)(26±7)%和6分钟步行距离(6MWT)(423±52)m,明显高于对照组的(39±8)%、(23±6)%和(361±32)m,差异均有统计学意义(均P<0.05);试药组治疗后PASP(36±9)mm Hg、hs-CRP(9±4)mg/L及NT-pro-BNP(535±49)ng/L均明显低于对照组的(40±8)mm Hg、(13±4)mg/L和(796±97)ng/L,两组间差异显著(均P<0.05)。结论西地那非治疗老年CHF并PH效果显著优于常规治疗。AIM To observe the efficacy of sildenafil in the treatment of elderly patients with chronic heart failure with pulmonary hypertension. METHODS 86 elderly patients in our hospital with chronic heart failure with pulmonary hypertension were randomly divided into two groups. The control group was treated with conventional therapy and the treatment group was treated with sildenafil. Left ventricular function, pulmonary arterial systolic pressure （PASP）, high-sensitivity C-reactive protein （hs-CRP） and N-terminal pro-brain natriuretic peptide （NT-pro-BNP） were compared between the two groups. RESULTS Left ventricular ejection fraction （LVEF） （42 ± 1 ） %, Left ventricular fractional shortening （LVFS） （26 ±7 ） % , and 6-minute walk test （6-MWT） （423 ± 52 ） m in the treatment group, were significantly higher than the control group （ 39 ±8 ） %, （ 23 ±6 ） %, （ 361 ± 32 ） m （ P 〈 0. 05 ） ; The levels of PASP （36 ±9） mmHg, hs-CRP （9 ±4） mg/L and NT-pro-BNP （535 ±49） ng/L in the obser- vation group were significantly lower than the control group （40 ± 8） mmHg, （13 ±4） mg/L, （796 ± 97 ） ng/L （ P 〈 0. 05 ）. CONCLUSION There are significant effects of sildenafil in the treatment of elderly patients with chronic heart failure with pulmonary hypertension and these effects are superior to conventional treatment.
