摘　　要：目的 探讨腕关节镜辅助下治疗复杂桡骨远端关节内粉碎性骨折的治疗方法及效果.方法 对2011年1月-2016年7月北京大学深圳医院收治的24例桡骨远端关节内粉碎性骨折患者,在腕关节镜辅助开放复位内固定术下手术治疗,术后随访评估腕关节功能恢复情况.结果 术后随访6~24个月.骨折愈合时间平均为10周.术后掌倾角9°~18°,尺偏角18°~28°,按照Sarmiento标准腕关节功能评定为优16例,良6例,一般2例.按照Lidstrom影像学评价指标优良率为87.5%.结论 应用腕关节镜辅助下治疗复杂桡骨远端关节内粉碎性骨折,不仅能达到骨折复位标准,手术创伤小,术后患者恢复快,还能减少骨折术后腕部功能障碍和腕关节不稳定的发生率.Objective To explore the clinical method and efficacy of wrist arthroscopy assisted treatment on complex intra -articular comminuted fracture of the distal radius. Methods The 24 patients with comminuted fracture of the distal radius in Shenzhen Hospital of Peking University were treated with surgery by wrist arthroscopy. Then we evaluated the function of wrist after surgery. Results Followed up the 24 cases for 6 to 24 months. The union time of fracture was about 10 weeks. After operation, the volar angle ware 9° to 18°, the radial inclination were 18° to 28°. According to Sarmiento criteria, 16 cases were graded as great, 6 cases were graded as good, and the rest 2 cases were graded as fair. According to lidstrom of radio appreciation, the excellent and good rate was 87.5%. Conclusion Wrist arthroscopy assisted treatment of complex intra-articular comminuted fracture of the distal radius, not only can achieve the fracture reduction criteria, less surgery trauma, quick postoperative recovery, but also can reduce the incidence of the wrist＇s dysfunction and the wrist joint instability.
