老年2型糖尿病患者糖化血红蛋白水平与牙周指数的相关性研究

摘　　要：目的:探讨老年2型糖尿病患者糖化血红蛋白控制水平对各项牙周指数的影响。方法:以256名老年2型糖尿病患者为研究对象,按照糖化血红蛋白控制目标分为2组,血糖控制水平良好组(≤8%)和血糖控制水平欠佳组(>8%),分析糖化血红蛋白变化对患者一般状况和探诊深度(PD)、附着丧失(AL)、牙龈指数(GI)、龈沟出血指数(SBI)、菌斑指数(PLI)、牙石检出率、根分叉病损等牙周指数的影响。结果:两组患者的年龄、刷牙习惯、教育水平、吸烟状况和BMI指数的构成分布基本相同(P>0.05),与血糖控制水平欠佳组相比,良好组患者的病程较短而经济收入较高(P=0.006,P=0.001)。牙周指数PD、PLI、牙石检出率在两组的比较中未见明显差异(P>0.05),而SBI、GI、AL和根分叉病损差异具有统计学意义(P=0.001,P=0.002,P=0.017,P=0.029)。结论:老年2型糖尿病患者糖化血红蛋白浓度越高,牙周组织的破坏程度越严重。Objective： To investigate the effect ofglycemic control on the periodontal index in elderly patients with type 2 diabetes mellitus （≥ 60 years）. Methods： 256 elderly patients with type 2 diabetes were selected and divided into 2 groups according to the value of glycosylated hemoglobin： good blood glucose control （≤ 8%） and poor blood glucose control （〉 8%）. Pocket depth （PD）, attachment loss （AL）, gingival index （GI）, sulcus bleeding index （SBI）, plaque index （PL1）, dental calculus detection rate and furcation involvement were observed. Results： The age distribution, brushing habits, educational level, cigarette consumption and BMI index were similar in all groups （P〉0.05）. Compared with the poor blood glucose control group, the patients in good group had shorter duration and higher economic income （P=-0.006, P=0.001）. In addition, there was no significant difference in PD, PLI, and calculus detection rate between good and poor groups （P〉0.05）, but SBI, GI, AL and root bifurcation were statistically significant （P=-0.001, P=-0.002, P=-0.0017, P=-0.029）. Conclusion： The higher the concentration of glycosylated hemoglobin in elderly patients with type 2 diabetes, the more severe destruction of periodontal tissue.
