真菌性角膜炎病原学及药敏结果分析

摘　　要：目的探讨真菌性角膜炎病原学分布特点及药敏结果。方法选择2006年1月-2016年12月医院眼科真菌性角膜炎200例作为研究对象,对200例患者眼角膜刮片标本进行真菌培养,行药敏检测。结果 200例真菌性角膜炎患者培养出221株病原真菌,以茄病镰刀菌、烟曲霉和光滑假丝酵母为主,分别为75株、23株和20株,分别占33.9%、10.4%、9.0%;75株茄病镰刀菌对伊曲康唑均不敏感,对伏立康唑敏感率为89.3%,对两性霉素B的敏感率为74.7%,对纳他霉素的敏感率为100.0%;23株烟曲霉对伊曲康唑、伏立康唑和纳他霉素的敏感率为100.0%,对两性霉素B的敏感率为52.2%;20株光滑假丝酵母对伊曲康唑、伏立康唑和纳他霉素的敏感率为100.0%,对两性霉素B的敏感率为85.0%。结论真菌性角膜炎的主要病原菌为茄病镰刀菌、烟曲霉和光滑假丝酵母,临床应选择敏感抗菌药物加以治疗。OBJECTIVE To explore the etiological characteristics of fungal keratitis and analyze the results of drug susceptibility testing.METHODS A total of 200 patients with fungal keratitis who were treated in department of ophthalmology from Jan 2006 to Dec 2016 were recruited as the study subjects,the corneal scraping specimens that were obtained from the 200 patients were cultured for fungi,and the drug susceptibility testing was performed.RESULTS Totally 221 strains of pathogenic fungi were isolated from the 200 patients with fungal keratitis,75（33.9%）of which were Fusarium solani,23（10.4%）were Aspergillus fumigatus,and 20（9.0%）were Candida glabrata.The 75 strains of F.solani were not sensitive to itraconazole,the drug susceptibility rate to voriconazole was 89.3%,the drug susceptibility rate to amphotericin B was 74.7%,and the drug susceptibility rate to natamycin was 100.0%.The drug susceptibility rates of the 23 strains of A.fumigatus to itraconazole,voriconazole and natamycin were 100.0%,and the drug susceptibility rate to amphotericin B was 52.2%.The drug susceptibility rates of the 20 strains of C.glabratato itraconazole,voriconazole and natamycin were 100.0%,and the drug susceptibility rate to amphotericin B was 85.0%.CONCLUSION F.solani,A.fumigatus and C.glabrata are the predominant species of pathogens isolated from the patients with fungal keratitis,and it is necessary for the hospital to choose sensitive antibiotics for treatment.
