摘  要:目的:探讨不同时期(心肌缺血前、心肌缺血时、心肌缺血再灌注时)给予多次短暂肢体缺血处理,对心肌缺血再灌注大鼠血清中心肌损伤标志物肌酸激酶MB同工酶(CK-MB)、心肌肌钙蛋白(cTnI)和心肌组织过氧化物酶体增殖物激活因子α(PPARα)mRNA和蛋白表达的影响。方法:40只健康雄性Wistar大鼠分为4组:缺血再灌注对照组(IR组)、心肌缺血期肢体处理组(LI-perC组)、心肌缺血前肢体处理组(LI-preC组)、心肌缺血后肢体处理组(LIpostC组),每组10只。试验结束后,各组大鼠留取血清和心肌组织标本,按照试剂盒说明检测CK-MB和cTnI含量,采用逆转录酶多聚酶链反应(RT-PCR)和Western blot方法检测心肌组织PPARαmRNA和蛋白表达水平。结果:与IR组相比,LI-perC组、LI-preC组、LIpostC组大鼠血清CK-MB和cTnI明显降低(P<0.05),心肌PPARαmRNA和蛋白表达均升高(P<0.05)。但以上指标在LI-perC组、LI-preC组、LI-postC各组间无明显差异(P>0.05)。结论:在大鼠心肌缺血再灌注不同时期给予多次短暂肢体缺血处理均可减轻心肌损伤,提高心肌PPARαmRNA和蛋白表达水平。心肌PPARα可能参与了肢体缺血处理对心肌缺血再灌注的保护过程。Objective:To observe the levels of CK-MB、cTnI and the expressions of peroxisome proliferator activated receptor-α(PPARα)mRNA and protein in myocardium of rats following myocardial ischemia-reperfusion(IR)injury and the impact of limb-ischemia(LI)conditioning for three times during different myocardial ischemic period.Methods:Fourty Wistar rats were randomly divided into 4 groups:IR group,LI-perC group(LI perconditioning),LI-preC group(LI precondioning),LI-postC group(LI postcondioning),ten rats in each group.Serum specimens and injured cardium of rats in four groups were kept after treatment.The contents of CK-MB and cTnI in the serum were detected according corresponding kits.The expressions of PPARα mRNA and protein in the myocardium were evaluated by reverse transcription polymerase chain reaction(RT-PCR)and Western blot respectively.Results:The levels of CK-MB and cTnI in LI-perC group,LI-preC group and LI-postC were significantly lower than those in IR group(P〈0.05),while the expressions of PPARαmRNA and protein in the three groups were significantly higher than those in IR group(P〈0.05).There were no significant changes in the contents of CK-MB and cTnI,the expressions of PPARαmRNA and protein in LI-perC group,LI-preC group and LI-postC group(P〉0.05).Conclusion:Limb-ischemia conditioning during different myocardial ischemic period may decrease myocardial injury and increase the expressions of PPARα mRNA and protein.
