针药并用治疗痛性糖尿病神经病变临床研究

摘　　要：目的观察针刺加内服加味活络效灵丹治疗痛性糖尿病神经病变(PDN)的临床疗效。方法 80例PDN患者随机分为治疗组和对照组各40例,均给予糖尿病常规治疗,治疗组给予针刺大陵、神门、血海、膈俞、足三里、三阴交等穴位,同时服用加味活络效灵丹;对照组给予维生素B1、弥可保口服。两组疗程均为1个月。结果治疗组总有效率87.50%高于对照组的55.00%。两组治疗前VAS疼痛评分差别不大(P>0.05)。治疗后治疗组VAS疼痛评分较治疗前有明显改善(均P<0.05),且治疗组优于对照组(P<0.05)。两组治疗前神经传导速度差别不大(P>0.05)。治疗后治疗组腓总神经、正中神经传导速度较治疗前改善(均P<0.05),且治疗组优于对照组(P<0.05)。结论针刺加内服加味活络效灵丹对PDN的疗效确切,且远期疗效稳定,优于西药。Objective： To observe the clinical effect of acupuncture combined with oral administration of modified Huoluo Xiaoling Decoction on Painful Diabetic Neuropathy（PDN）. Methods： 80 patients were randomly divided into the treatment group and the control group. Both groups received regular treatment of diabetes. The treatment group was given oral administration of modified Huoluo Xiaoling Decoction and acupuncture in Daling（PC7）,Shenmen（HT7）,Xuehai（SP10）,Geshu（BL17）,Zusanli（ST36）,Sanyinjiao（SP6） and so on,while the control group was given oral administration of victim B1 and Methyleobalamin. The treatment course was one month. Results： The total effective rate of the treatment group was 87.50%,higher than that of the control group（55.00%）. There was no significant difference in the pain scores between the two groups before the treatment（P〈0.05）. After treatment,the pain score of VAS in the treatment group was significantly improved,compared with that before treatment（P〈0.05）,and that in the treatment group was better than that in the control group（P〈0.05）. There was no significant difference in the nerve conduction velocity between the two groups before the treatment（P〈0.05）. After treatment,the conduction velocity of nervus peroneus communis and nervi medianus in the treatment group was better than that before treatment（P〈0.05）,and that in the treatment group was better than that in the control group（P〈0.05）. Conclusion： Acupuncture combined with oral administration of modified Huoluo Xiaoling Decoction has significant and steady long-term efficacy,which is superior to Western medicine.
