主动脉内球囊反搏后发生血小板减少症的危险因素及对近期预后影响的临床分析

摘　　要：目的分析及探讨临床应用主动脉内球囊反搏后发生血小板减少症的危险因素及对近期预后的影响。方法选取我院2014年3月~2016年3月的108例接受主动脉内球囊反搏治疗患者,根据是否发生血小板减少症分为血小板减少组(74例)及非血小板减少组(34例),回顾性分析入选患者的血小板情况及临床特点。结果 74例患者(68.5%,74/108)发生血小板减少症,血小板减少程度随球囊反搏时间延长而加重。多因素Logistic回归分析显示,基线血小板计数是本研究中唯一具有统计学差异的影响因素(P<0.01),两组的死亡率及30 d再住院率比较差异无统计学意义(P>0.05)。结论血小板减少症的程度与球囊反搏应用时间相关,较低的基线血小板计数是发生血小板减少症的独立危险因素,血小板减少症不影响疾病的近期预后。Objective To analyze and explore the risk factors of thrombocytopenia and its influence on the short-term prognosis in patients undergoing intra-aortic balloon pump （IABP） counterpulsation.Methods 108 patients who received IABP in our hospital from March 2014 to March 2016 were selected and divided in[o the thromboeytopenia group （74 cases） and the non thrombocytopenia group （34 cases） according to whether thrombocytopenia occurred.The platelet and clinical features of the selected patients were analyzed retrospeetively.Resdts Thrombocytopenia occurred in 74 pa- tients （68.5%,74/108）,and the degree of thrombocytopenia increased with the duration of IABP support.Multi factor Lo- gistic regression analysis showed that baseline platelet count was the only influential factor in this study （P〈0.01）,and there was no significant difference between the two groups in mortality and 30 days re-admission rate （P〉0.05）.Conclusion The degree of thrombocytopenia is related to the duration of IABP support.Lower baseline platelet count is independent risk factor for thrombocytopenia,which is not affect the short-term prognosis.
