摘  要:目的探讨膝关节骨性关节炎感染的危险因素及相关病原菌特点。方法回顾性分析随机抽取720例膝关节骨性关节炎患者的病历资料,以及感染患者的病原菌及耐药性。结果 720例患者中,感染的有60例,共分离出96株病原菌,其中革兰阳性菌46株,占47.92%;革兰阴性菌40株,占41.67%。金黄色葡萄球菌和表皮葡萄球菌对青霉素、苯唑西林完全耐药,未检出对万古霉素和利奈唑胺的耐药菌;主要革兰阴性菌对头孢类有较高的耐药性,其中大肠埃希菌对氨苄西林、头孢他啶完全耐药,对亚胺培南、美罗培南敏感率较高。多因素分析显示,肥胖、高血压、糖尿病是出现感染的危险因素(P<0.05)。结论膝关节骨性关节炎感染以金黄色葡萄球菌为主,感染的危险因素多,临床需加强防护措施。Objective To analyze the risk factors of knee osteoarthritis infection and observe characteristics of related pathogens. Methods To retrospectively analyze the medical records of 720 cases of knee osteoarthritis,as well as pathogens and drug resistance of the infected patients. Results Of the 720 patients,60 were infected with 96 pathogenic bacteria,of which46 were gram-positive bacteria,accounting for 47. 92%; 40 were gram-negative bacteria,accounting for 41. 67%. Staphylococcus aureus and Staphylococcus epidermidis were completely resistant to penicillin and oxacillin,and no resistant strains to vancomycin and linezolid were detected. The main gram-negative bacteria had high resistance to cephalosporins,of which Escherichia coli was completely resistant to ampicillin and ceftazidime,and had a high sensitivity to imipenem and meropenem. Multivariate analysis showed that obesity,hypertension and diabetes were the risk factors of infection( P〈0. 05). Conclusion Infection of knee joint osteoarthritis is mainly Staphylococcus aureus with many risk factors of infection,so protective measures should be strengthened in clinical.
