摘  要:目的探讨手足口病患儿血管内皮素(endothelin1,ET-1)、C反应蛋白(creactiveprotein,CRP)、白细胞介素_6(interleukin6,IL-6)水平变化的临床意义。方法选取2015年1月至2016年11月在湖南省儿童医院接受治疗手足口病患儿120例。根据患儿临床特征,分为普通组39例、重症组60例与危重症组21例。观察并对比三组患儿心率、血白细胞计数、血糖、血清ET-1、CRP、IL-6水平变化。结果危重症组患儿比较普通组和重症组年龄偏小、血糖偏高、心率偏快。危重症组患儿血白细胞计数高于普通组;重症组患儿心率、血白细胞计数、血糖均较普通组显著升高。危重症组患者血清中ET-1的水平与其它两组比较明显升高。重症组患儿入院1、3、7d时CRP、IL-6因子与普通型相比水平明显升高;危重症组患儿入院1d、3d、7d时CRP、IL-6;因子水平明显高于普通型和重症型。多因素分析发现,ET-1、CRP、IL-6均是病情程度的独立危险因素(P〈0.05)。结论手足口病患儿血清中IL-6、CRP以及ET-1水平的检测可以对患者病情的判断起到辅助的作用。Objective To explore the clinical significance of endothelin 1 ( ET-1 ), creactive protein (CRP) and interleukin-6 (IL-6) levels in children with hand-foot-and mouth disease. Methods All 120 chil- dren with hand-foot-and mouth disease treated in our hospital from January 2015 to November 2016 were recruited for this study. According to the clinical characteristics, patients were divided into normal group ( n = 39 ), se- vere group ( n = 60), and critical group ( n = 21 ). Heart rate, white blood cell count, blood glucose, serum ET- 1 ,serum CRP, IL-6 levels of the patients were observed. Results Compared with normal group and severe group ,patients in the critical group were characterized with young age, high blood sugar, and fast heart rate. White blood cell count of the patient in critical group is higher than that of normal group. Children in severe group showed severely elevated heart rate, white blood cell count, blood glucose levels, compared to the patients of normal group. ET-1 of the critically ill patients was significantly increased compared with that of the patients in other groups. Children in the critical illness group showed higher levels of CRP after the admission at day 1, 3 ,and 7. ET-1, CRP, and IL-6 were independent risk factors for hand-foot-and mouth disease patients( P 〈 0. 05 ). Conclusions The detection of serum IL-6, CRP, and ET-1 can provide useful information for the clini- cal judgement on the children with hand-foot-and mouth disease.
