Th1、Th2型细胞因子在肺癌合并慢性阻塞性肺疾病患者临床应用价值

摘　　要：目的探讨Th1、Th2型细胞因子在肺癌合并慢性阻塞性肺疾病患者中的临床应用价值。方法选取自2016年4月至2017年11月收治的80例肺癌患者为研究对象。将40例肺癌合并慢性阻塞性肺疾病(COPD)患者设为LC-COPD组,同期收治的未合并COPD的40例患者设为LC组,比较两组患者外周血清中白细胞介素(IL)-2、干扰素-γ(IFN-γ)、肿瘤坏死因子-α(TNF-α)、IL-4、IL-6、IL-10的表达水平,Th1、Th2细胞比例及两种细胞比值。结果 LC-COPD组患者血清中IL-2、TNF-α、IL-10水平明显高于LC组,IL-4、IL-6水平明显低于LC组,两组间比较,差异均有统计学意义(P<0.05);两组IFN-γ水平比较,差异无统计学意义(P>0.05)。LC-COPD组和LC组中,Th1/Th2比值分别为(5.99±0.22)和(4.62±0.93),两组间比较,差异有统计学意义(P<0.05)。结论肺癌合并COPD患者血清中Th1细胞亚群及其细胞因子的表达水平较高,提示Th1/Th2比值可以作为预测疾病预后的潜在指标。Objective To investigate the clinical value of Thl and Th2 cytokines in patients with lung cancer combined chronic ob- structive pulmonary disease （COPD）. Methods A retrospective study was performed on 80 cases of patients with lung cancer com- bined COPD who were admitted from April 2016 to November 2017. The patients combined with COPD were divided into the LC-COPD group （ n = 40）, the other patients not combined with COPD were divided into the LC group （ n = 40 ）. The peripheral blood of serum in- terleukin-2（IL-2） and interferon-γ（IFN-γ） ,tumor necrosis factor-α（TNF-α） ,the expression levels of IL-4 ,IL-6 and IL-10,Thl and Th2 cells proportion and the ratio of two kinds of cells were compared between the two groups of patients. Results The serum levels of IL-2 ,TNF-α and IL-10 in the LC-COPD group were significantly higher than those in the LC group,while the levels of IL-4 and IL-6 were significantly lower than those in the LC group, and the differences between the two groups were statistically significant （ P 〈 0. 05 ） ; there was no statistically significant difference in level of IFN-γ between the two groups （ P 〉 0. 05 ）. In the LC-COPD group and LC group,Th1/Th2 ratio was （5. 99 ：tO. 22） and （4. 62 ＋0. 93） ,respectively;the differences between the two groups were statis- tically significant （ P 〈 0. 05 ）. Conclusion The expression level of Thl cell subgroup and cytokines in the serum of patients with lung cancer combined with COPD is higher,indicating that Thl/Th2 ratio could be used as a potential indicator to predict the prognosis of the disease.
