摘　　要：探讨RA并肺间质纤维化患者中TGF-β、KL-6的水平变化及其临床意义。选取确诊RA并肺间质纤维化患者30例作为研究组,同期单纯RA患者60例作为对照组,检测并比较两组血清TGF-β、KL-6水平,并分析TGF-β、KL-6与肺间质纤维化相关指标的关系以及与HRCT的相关性。结果显示,研究组的血清TGF-β、KL-6水平显著的高于对照组,研究组的RF、CRP、中性粒细胞检测值显著的高于对照组,研究组和对照组的血沉检测值差异无统计学意义;并且研究组TGF-β、KL-6水平与患者RF、CRP呈显著的正相关,与血沉及外周血中性粒细胞水平无显著相关性;研究组HRCT评分为（11.6±3.6）分,研究组患者TGF-β、KL-6水平与HRCT评分均呈显著的正相关。说明除血沉和外周血中性粒细胞水平外,其余指标都可作为RA合并肺间质纤维化的判定依据,为RA合并肺间质纤维化的早期诊断提供了参考依据,具有重要的临床意义。This research studied changes of TGF-βand KL-6 levels in patients with RA and pulmonary interstitial fibrosis and their clinical significance.30 patients diagnosed with RA and pulmonary interstitial fibrosis were selected as study group,at the same period,60 patients only with RA were selected as control group.TGF-βand KL-6 levels in the serum of two groups were detected and compared,so as to analyze the relationship between TGF-β,KL-6 and indexes related to pulmonary fibrosis and the correlation with HRCT.The results showed that TGF-βand KL-6 levels in the serum of study group were significantly higher than those in the control group,RF,CRP,neutrophil test values in the study group were significantly higher than those in the control group,and the difference in erythrocyte sedimentation rate（ESR）detection between study group and control group had no statistical significance;TGF-βand KL-6 levels in the study group were significantly positive correlated with RF and CRP in patients,but had no significant correlation with ESR and peripheral blood neutrophil level;HRCT score in the study group was（11.6±3.6）,TGF-βand KL-6 levels were significantly positive correlated with HRCT scores.It was indicated that except ESR and peripheral blood neutrophil level,the other indexes could be used as the basis for determining RA with pulmonary fibrosis,which might provide a reference for early diagnosis of RA and pulmonary interstitial fibrosis and have important clinical significance.
