清热解毒化湿法治疗膜性肾病研究进展

摘　　要：对膜性肾病的病因病机进行系统考证,认为古代中医文献中没有"膜性肾病"病名记载,按其泡沫尿、四肢浮肿、腰酸、乏力等临床症状,可归属于"尿浊""水肿""腰痛""虚劳"等病范畴。病因方面研究认为,内因多由禀赋不足、年老体弱、饮食不节或情志内伤等致正气亏虚;外因多由湿热之邪侵袭人体,内外合邪致病。该病的病因病机核心是脾肾两虚,本虚标实,以虚为主。盖肾为先天之本,脾为后天之本,前者运化水湿,后者蒸腾气化,标实则以湿热(毒)为主。机体脾肾虚弱,则蒸腾运化不利,从而湿热(毒)之症状愈发加重,法当清热解毒、化湿治疗。治疗方面总结现代医家的治疗方法,采用单味药、中成药和专方治疗等方法治疗,疗效显著。After a systematic examination of the etiology and pathogenesis of membranous nephropathy,no recording of the disease name of membranous nephropathy has been found in the literatures of traditional Chinese medicine.According to the clinical symptoms of foamed urine,edema in the extremities,soreness of waist,lack in strength,etc.,this disease is categorized as ＂turbid urine＂,＂ edema＂,＂ backache＂,＂ consumption＂,etc.Researches in etiology indicate that,in terms of internal causes,deficiency of natural endowment,advanced age and weak body,improper diet,and emotional injuries have caused the deficiency of positive Qi;in terms of external causes,the pathogen of heat-damp intrudes into human body;combination of the internal and external causes leads to disease.The core of the etiology and pathogenesis of membranous nephropathy lies in the deficiency of spleen and kidney.Asthenia in origin is accompanied by sthenia in superficiality,but asthenia prevails.Kidney,as the origin of congenital constitution,conveys and transforms damp;spleen,as the origin of acquired constitution,transpires damp.For sthenia in superficiality,heat-damp（toxin） prevails.For the body with deficient and weak spleen and kidney,the conveyance,transformation and transpiration are not smooth,thus heat-damp（toxin） symptom aggravates.Treatment methods of heat clearing,toxin removing and damp eliminating shall be adopted.Treatment methods of single-ingredient Chinese herbal medicine,Chinese patent drug,and special prescriptions in combination with modern medicine will achieve a remarkable affect.
