基于现代医案的黄连阿胶汤证治规律研究

摘　　要：目的:探讨黄连阿胶汤在现代临床中的方证特点和运用规律,为古方今用提供思路和方法。方法:以"黄连阿胶汤"为主题词,计算机检索中国知网(CNKI)、维普(VIP)、中国生物医学文献数据库(CMB)三大数据库,年限为建库至2017年7月。对符合要求的文献用Excel建立数据表,利用SPSS22.0统计软件进行统计分析。结果:纳入122篇文献,共130例病案。经统计发现,临床应用中,常见中医病名:不寐、口疮、崩漏;西医病名:失眠症、复发性口腔溃疡、功能性子宫出血。基本病机为肾阴亏虚,心火亢胜。临床多在原方基础上加味,多加味生地黄、酸枣仁、牡蛎3味药物。结论:当代医家应用黄连阿胶汤的基本指征是失眠、烦躁、口燥咽干、舌红少苔、脉细数。Objective： To explore the characteristics in formula and syndrome and the application rules of Huanglian Ejiao Decoction in modern clinical practice to provide new idea and method to serve the present clinical performance. Methods： Huanglian Ejiao Decoction was retrieved as a keyword by computers in three major datebases,namely,CNKI,VIP,and CBM from the establishment date of each of them to July 2017. The articles which meet the requirements were input Excel table and then were used by SPSS22.0 software for statistical analysis. Results： A total of 122 papers meet the inclusion criteria,including 130 medical cases within them. The statistics suggest that Huanglian Ejiao Decoction are commonly used in treating for insomnia,aphtha,metrorrhagia in TCM and are commonly used in treating for insomnia,recurrent oral ulcer,and functional uterine bleeding in western medicine. Its basic pathogenesis of it application is Kidney Yin Deficiency and hyperactivity of Heart Fire. The original formula are often used usually by adding three kinds of herbs,namely,Radix Rehmanniae,Semen ziziphi spinosae and Concha ostreae. Conclusion： The basic physical indications of Huanglianejiao Decoction being used by contemporary doctord are insomnia,dysphoria,thirst,red tongue,little coating as well as weak and fast pulse.
