肤悦康洗剂联合补脾祛风方对特应性皮炎患者Th17、IL-17和IL-23的影响  

Effect of Fu Yue Kang Lotion Combined with Bupi Qufeng Prescription on Th17,IL17 and IL-23 in Patients with Atopic Dermatitis

摘　　要：目的:观察肤悦康洗剂联合补脾祛风方治疗特应性皮炎的临床疗效及其对特应性皮炎辅助性T细胞17(T helper cell 17,Th17)、白细胞介素-17(interleukin-17,IL-17)及白细胞介素-23(interleukin-23,IL-23)细胞通路的影响。方法:使用随机、对照、单盲的方法,将入选115例特应性皮炎患者分为治疗组60例和对照组55例,治疗组采用肤悦康洗剂(苦参、白矾、土槿皮、地肤子、白鲜皮、荆芥、广东紫荆皮)湿敷患处联合补脾祛风方(党参20g,白术10g,炙甘草10g,黄芪20g,白蒺藜15g,茯苓15g,防风10g,蝉蜕10g)内服治疗;对照组外用尿素乳膏联合口服复方甘草酸苷片+氯雷他定片治疗。两组患者均治疗2周。观察两组患者特应性皮炎严重程度评分指数(scoring atopic dermatitis index,SCORAD)积分变化情况,并检测患者外周血及皮损中Th17、IL-17及IL-23水平。结果:治疗组在治疗后SASSAD积分下降,与本组治疗前比较及治疗后两组患者比较,差异均有统计学意义(P<0.01);治疗组患者治疗后外周血及皮损中Th17、IL-17及IL-23水平明显下降,与同组治疗前比较,且与两组患者治疗后比较,差异均有统计学意义(P<0.01)。结论:肤悦康洗剂联合补脾祛风方治疗特应性皮炎可显著改善患者临床症状,降低皮损SASSAD积分,降低患者外周血及皮损中Th17、IL-17及IL-23水平。Objective： To investigate the effect of Fu Yue Kang Lotion combined with Bupi Qufeng Prescription in treatment of atopic dermatitis and its effect on Th17,IL-17 and IL-23 in cells in patients with atopic dermatitis. Methods： By using randomized,controlled,single blind method,the selected 115 patients with AD were divided into treatment group（60 cases） and control group（55 cases）. The treatment group was treated with Fu Yue Kang Lotion（Sophora,Alum,Pseudolarix,Kochia scoparia,Cortex dictamni,Nepeta,Guangdong Bauhinia） combined Bupi Qufeng Prescription（Codonopsis 20 g,Atractylodes 10 g,Baked Licorice 10 g,Astragalus 20 g,Tribulus terrestris 15 g,Poria 15 g,radix sileris10 g,Cicada slough 10 g）,while the control group was treated with Urea cream and Glycyrrhizic acid glucoside tablets ＋ chlorine thunder he stator. The treatment lasted for 2 weeks. The changes of SASSAD score in both groups were observed,and the levels of Th17,IL-17 and IL-23 in peripheral blood and skin lesions were detected. Results： The SASSAD score of the treatment group signifcantly decreased after treatment（P〈 0. 01）. Compared with that before treatment and with that of the control group,IL-23,IL-17 and Th17 levels in the treatment group were significantly decreased（P〈 0. 01）. Conclusion： Fu Yue Kang Lotion combined with Bupi Qufeng Prescription can significantly improve symptoms,reduce skin SASSAD integral and decrease Th17 and IL-17 and IL-23 in blood and skin in treatment of atopic dermatitis（AD）,which is worth clinical promotion.
