参苓白术散联合西药治疗慢性肾小球肾炎(脾气虚)随机平行对照研究  

Shenling Baizhu Powder combined with Western Medicine Treatment of Chronic Glomerulonephritis(Piqixu) Randomized Controlled Study

摘　　要：[目的]观察参苓白术散联合西药治疗慢性肾小球肾炎(脾气虚)疗效。[方法]使用随机平行对照方法,将64例住院及门诊患者按随机数字表方法随机分为两组。对照组32例强的松,1mg/kg,晨起顿服;力平脂,200mg/次,1次/d;环磷酰胺,100mg/次,2次/d;潘生丁,75mg/次,3次/d。治疗组32例参苓白术散(人参、茯苓各15g,炒白术20g,山药30g,炒白扁豆、莲子、炒薏苡仁各15g,砂仁6g,桔梗9g,甘草6g,水肿明显加陈皮、苍术各12g,大腹皮15g;畏寒明显加干姜12g,肉桂、制附子各6g;腰膝酸软明显加菟丝子、肉苁蓉各30g,仙鹤草20g;血尿加小蓟12g,阿胶15g,蒲黄9g;五心烦热加知母、黄柏各9g,玄参12g),1剂/d,水煎200m L,早晚分服;西药治疗同对照组。连续治疗30d为1疗程。观测临床症状、尿红细胞、24h尿蛋白、不良反应。治疗1疗程(30d),判定疗效。[结果]治疗组显效15例,缓解8例,有效6例,无效3例,总有效率90.62%;对照组显效11例,缓解6例,有效7例,无效8例,总有效率75.00%;临床疗效两组无显著差异(P>0.05)。尿红细胞,24h尿蛋白定量两组均有改善(P<0.01,P<0.05),治疗组改善优于对照组(P<0.05,P<0.01)。[结论]参苓白术散联合西药治疗慢性肾小球肾炎(脾气虚),疗效满意,无严重不良反应,值得推广。[Objective] To observe shenlingbaizhupowder combined with western medicine treatment of chronic glomerulonephritis （Piqixu） effect. [Methods] 64 cases of inpatients and outpatients were randomly divided into two groups according to the random number table method by random parallel control. Control group, 32 cases of prednisone, lmg/kg, morning clothing, Liping fat, 200 mg/times, 1 times/d; cyclophosphamide, 100 mg/times, 2 times/d; dipyridamole, 75 mg/times, 3 times/d. The treatment group of 32 cases （Renshen, Fuling aech 15g, Chaobaizhu 20g, Shanyao 30g, Chaohaibiandou, Lianzi, Chaoyiyiren each 15g, Sharen 6g, Jigeng 9g, Ganeao 6G, obvious edema add Chenpi, Cangzhu each 12g, Dafupi 15g, chills add Ganjing 12g, Rougui, Zhifuzi each 6g, soreness and weakness of waist and knees add Tusizi, Roucongrong each 30g, Xianhecao 20g; hematuria add Xiaoji12g, Ejiao15g, Puhuang 9g, Fanre add Zhimu, Huangbaieach 9g, Xuanshen 12g）, 1 agent/d, decoction 200mL, sooner or later the service; westernmedicinewiththecontrolg roup. The continuous treatment of 30d was 1 courses. The clinical symptoms, urine red blood cells, 24h urine protein, adverse reactions were observed. The 1 course of treatment （30d） was treated and the curative effect was determined. [Results] In the treatment group, 15 cases were markedly effective, 8 eases were relieved, 6 cases were effeetive, 3 cases were invalid, the total effective rate was 90.62%. In the eontrol group, 11 cases were markedly relieved, 6 cases were relieved, 7 eases were effective, 8 cases were ineffective, the total effective rate was 75%, and there was no signifieant difference in clinical efficacy between 2 groups （P〉 0.05）. Urine red blood cells, 24h urine protein quantitative two groups were improved （P〈0.01, P〈0.05）, the treatment group was better than the control group （P〈0.05, P〈0.01）. [Conclusion] Shenling Baizhu powder combined with western medicine treatment of chronic glomerulonephritis （Piqixu）, satisfactory curative effect,no serious adve
