硬化性肺泡细胞瘤的CT表现及鉴别诊断

摘　　要：目的分析硬化性肺泡细胞瘤（PSP）的螺旋CT表现，提高其诊断准确率。方法回顾性分析23例经病理证实的PSP（25个病灶）的临床及胸部CT资料。结果CT平扫均表现为肺内单发圆形或类圆形结节或肿块，2例可见浅分叶，13例密度均匀，5例伴点状钙化；增强扫描大多呈中度至明显强化，15例增强扫描患者中，10例呈明显均匀或不均匀强化。伴随征象主要有贴边血管征9例，病变周围晕征2例，空气新月征1例，尾征1例。结论PSP的CT表现具有良性肿瘤特点，形态分布、强化方式及伴随征象有一定的特征性，综合分析其CT征象有助于提高诊断准确率。Objective To analyze the spiral CT findings of pulmonary sclerosing pneumocytoma （PSP）, in order to improve the diagnostic accuracy. Methods CT data of 23 （with total 25 lesions） patients confirmed PSP with pathology were retrospectively analyzed. Results Under unenhanced CT, all pulmonary lesions presented as round or oval shaped nodule or mass. 2 cases showed slight lobulation. 13 lesions were even in density, and 5 lesions had scattered spot calcification. After contrast CT scan, most of the lesions showed moderate-marked enhancement. Of the 15 cases, heterogeneous or homogeneous enhancement was assessed in 10 cases. Associated findings were welt vessel sign （n=9）, a halo of ground glass shadow around lesion margins （n=2）, air meniscus sign （n=1） and tail sign （n=1）. Conclusion The CT findings of PSP has the characteristics of benign tumors, and there have certain characteristics in shape distribution, enhancement form and adjoint features. A comprehensive analysis of the CT features can help to improve the accuracy of diagnosis.
