体外循环辅助下难治性纵隔肿瘤外科治疗的护理对策

摘　　要：目的初步总结体外循环辅助下累及心脏或大血管的难治性纵隔肿瘤外科治疗的护理对策。方法回顾性分析2008年1月-2016年12月实施的体外循环下外科手术切除累及心脏或大血管的9例纵隔肿瘤患者，总结患者的一般情况、临床特征、治疗方法及护理。结果9例患者中纵隔肿瘤全部切除者7例，大部分切除者2例；2例同时行肺动脉修补术，3例同时行部分心包切除术，3例同时行肺楔形切除术。所有患者围手术期均得到有针对性、个性化护理，术后症状均得到有效缓解出院。结论对于累及心脏或大血管的难治性纵隔肿瘤可以实施体外循环辅助下的外科手术治疗，借用体外循环技术，可以提高难治性纵隔肿瘤的手术切除率，及时有效的围手术期护理干预有利于患者的顺利康复。Objective To summarize the nursing measures taken after the surgical treatment of those patients with the refractory mediastinal tumor adhering to the heart or great vessels with the aid of cardiopulmonary bypass. Methods We retrospectively analysed 9 cases of patients with refractory mediastinal tumor who were operated from January 2008 to December 20t6 by aid of cardiopulmonary bypass in our department. The general situation, the clinical features, treatment methods and nursing of these patients were summarized. Results The tumors were resected completely in 7 cases, subtotal resection of tumors were performed in 2 cases; 2 cases of pulmonary artery repair, 3 cases of partial pericardial resection and 3 cases of pulmonary wedge resection were done at the same time. All patients were recovered after all targeted, individualized nursing methods were adopted. Conclusion The refractory mediastinal tumors can be resected under cardiopulmonary bypass. Timely effective perioperative nursing intervention is helpful to the rehabilitation of those patients.
