摘　　要：目的研究冷冻球囊消融治疗阵发性心房颤动(房颤)围术期并发症,并分析房颤早期复发的预测因素。方法选择2014年3月至2016年12月于武汉大学人民医院心内科接受冷冻球囊消融治疗阵发性房颤患者30例,其中男性14例,女性16例;年龄52~70岁,平均年龄60.5岁。行冷冻球囊消融治疗。分析围术期发生的并发症。随访冷冻球囊消融术后早期(3个月内)房颤复发的情况,并分析复发的预测因素。结果 30例患者均成功完成手术,无严重并发症发生。术中发生迷走神经反射2例(6.7%),1例于左上肺静脉冷冻复温后发生,1例于左下肺静脉冷冻复温后发生。无心包积液、膈神经麻痹等事件发生。术后随访3个月,其中7例(23.3%)患者在术后早期复发房颤,研究显示左心房内径是房颤患者行冷冻球囊消融术后早期复发的预测因素。结论冷冻球囊消融治疗阵发性房颤成功率高,围术期及近期随访安全性良好。左心房内径是阵发性房颤患者冷冻球囊消融术后早期复发的预测因素。Objective To investigate the perioperative complications of cryoballoon ablation for paroxysmal atrial fibrillation（AF）, and analyze the predictors of recurrence of AF in the short term. Methods From March 2014 to December 2016, a total of 30 paroxysmal AF patients performed by cryoballoon ablation were enrolled, which included 14 males and 16 females, aged52-70 years old with mean age of 60.5 years old. The perioperative complications were analyzed, and assessed the predictors on early stage（within 3-month） recurrence of AF after cryoballoon ablation. Results All of 30 cases were successful without severe complications. The vagal reflex in 2 cases（6.7 %）, and 1 was in left superior pulmonary vein（LSPV） and 1 was in left inferior pulmonary vein（LIPV）. There was no hydropericardium, phrenic nerve paralysis and other events. All of them were followed up 3-month. Among these caes, 7 cases（23.3 %） were early recurrence of AF, which shown that the size of left atrium was the predictor of early recurrence after balloon ablation in AF patient. Conclusion It is demonstrated that cryoballoon ablation showed the high success rate in treatment of paroxysmal AF, which is safe during perioperative and early postoperative periods.The left atrial diameter is the predictor of early recurrence after cryoballoon ablation in patients with paroxysmal AF.
