摘　　要：目的分析自动出院患者出院时的状况,为制定对策提供依据。方法利用病案首页信息表和医嘱信息表查询出2016年所有下达"自动出院"医嘱的患者351例,根据ID号逐份提取完整病案,查阅病案中记载的患者出院时的状况及相关信息进行分析。结果将351例自动出院患者出院状况分为3组,好转出院者占22.5%,恶化出院者占39%,濒死出院者占38.5%。其中恶化、濒死自动出院患者中,均以循环系统疾病居首。患者自动出院原因主要有5种,即患者预后差、家属治疗意愿低,患者治疗花费大、家庭经济条件差,外地住院照顾不方便、要求回当地治疗,患者对疾病认识不足、认为已治愈和患者担心影响工作。结论面对自动出院的患者,医护人员应根据患者自动出院时的状况,在充分告知的基础上,进行适度劝阻,同时加大宣传教育力度,积极提供相关医学建议,将自动出院可能给患者造成的伤害降到最低。Objectives This paper analyzed the discharge status of discharged patients, and provided evidence for further development of countermeasures. Methods Attempts to use information table and formula information table query out of all 2016 issued ＂automatic discharge＂ orders of 351 cases of patients, and then according to the ID number by extract complete medical history, the status of the inspection of the medical records of patients at discharge and related information for analysis. Results According to the 351 patients who were discharged from the hospital automatically, they were divided into three groups, which improved the number of patients in the hospital by 22.5 percent, and the number of patients who had worsened the hospital was 39 percent, and the number of those who died was 38.5 percent. Among them, the deterioration and dying automatic discharge patients, with circulatory disease topped. Patients discharged automatically reason mainly has five, the patients with poor prognosis, low family care, patient treatment cost of large, poor family economic condition, field hospital care convenient, not ask back to local treatment, insufficient understanding of disease, patients have been cured and patients worry affects my work. Conclusions Face automatic discharge side, medical personnel should be discharged automatically according to patients condition, on the basis of fully informed, doing moderate, intensify propaganda and education at the same time, actively provide medical advice, will automatically discharge may give patients caused by damage to a minimum.
