摘　　要：目的调查某单位接种乙肝疫苗发生疑似预防接种异常反应的原因。方法对6例病例临床资料进行分析,开展疑似预防接种异常反应和食源性疾病的现场流行病学调查,采集15件留样食品进行金黄色葡萄球菌、副溶血性弧菌、沙门氏菌、大肠菌群和肉毒杆菌检测;采集10支同批次乙肝疫苗进行外观、无菌试验、鉴别试验、异常毒性试验和细菌内毒素检测;采集5支同批次一次性注射器进行无菌试验和细菌内毒素检测。结果 6例病例年龄最大20岁、最小18岁,均为男性。临床表现主要以恶心、腹痛为主,无明显阳性体征,且症状相似,结合流行病学调查、实验室检测结果和发病经过分析,诊断为群体性心因性反应事件,经心理治疗和对症处理,6例病例当日痊愈。结论本次事件为接种乙肝疫苗发生群体性心因性反应所致。建议加强宣传教育,规范预防接种门诊建设和管理。Objective To investigate the cause of Adverse Events Following Immunization by hepatitis B vaccine on a unit. Methods The clinical data of 6 cases were analyzed,epidemiological survey on the field was performed including Adverse Events Following Immunization and foodborne diseases. Samples of 15 samples were collected for detection of Staphylococcus aureus,Vibrio parahaemolyticus,Salmonella,coliforms and Clostridium botulinum; 10 batches of hepatitis B vaccine were collected for the appearance,asepsis test,differential test,abnormal toxicity test and bacterial endotoxin test; 5 single syringes were collected for asepsis test and bacterial endotoxin test. Results There are 6 cases in the incident,the oldest was 20 years old,and the youngest was 18 years old,all were male. The patient had nausea and abdominal pain,with no obvious positive signs and similar symptoms,through the analysis of clinical manifestations and laboratory test show that This is a psychogenic reaction event,after psychological treatment and symptomatic treatment,6 patients recovered the same day. Conclusion The conclusion was determined as a case of psychogenic reaction by hepatitis B vaccination. It is suggested that publicity and education should be strengthened and the construction and management of vaccination clinics should be standardized.
