摘　　要：目的采用2%利多卡因注射液局部注射联合加巴喷丁口服治疗带状疱疹后遗神经痛(PHN),观察其有效性和安全性。方法将100例PHN患者随机分为2组,每组50例,分别接受加巴喷丁口服(A组)和2%利多卡因局部注射联合加巴喷丁口服治疗(B组),共治疗10 d。用视觉模拟评分(VAS)和24 h睡眠时间来评价治疗效果,同时观察并发症及药物不良反应。结果在10 d的观察期间,两组患者治疗后各时点与治疗前相比疼痛评分随时间下降,睡眠时间均增加(P<0.05)。B组的VAS评分下降大于A组,24 h睡眠时间增加大于A组(P<0.05)。两组未出现并发症及严重的药物不良反应。结论 2%利多卡因注射液局部注射联合加巴喷丁口服治疗PHN,可迅速缓解疼痛,改善睡眠质量,疗效确切,效果优于单独加巴喷丁口服,且无明显不良反应。Objective To study the effect and safety of local injection of 2% lidocaine combined with gabapentin in the treatment of post-herpetic neflralgia（PHN）. Methods A total of 100 patients with PHN were randomly divided into two groups： Gabapentin group （ group A, n=50） and local injection of 2% lidocaine combined with gabapentin group （ group B, n=50）. The effects were valuated by visual analogue scale （VAS） and according to 24-hour sleeping time, and the side effects and complications were also observed. Results The pain scores after the treatment decreased with time compared with those before treatment in 2 groups during 10 days, both the decrease of VAS and the sleeping time within 24 hours were higher in group B （P〈0.05）. Compared with group A, sleeping time in 24 hours increased more in group B （P〈0.05）. Side effects and complications were not found in the two groups. Conclusion The strategy of local injection of 2% lidocaine combined with gabapentin is effective and safe for the treatment of PHN, which is better than oral gabapentin alone.
