国产封堵器对动脉导管未闭术左心功能的影响

摘　　要：目的研究国产动脉导管未闭(PDA)封堵器治疗对左侧心房心室的大小和收缩功能的影响。方法 2014年3月至2016年10月聊城市第二人民医院诊治的40例PDA患者,均采用国产封堵器和输送系统进行封堵手术。患者在介入术前和术后1 d、3个月时进行超声心动图检查,测量PDA患者的左侧心室舒张末期内径(LVDd)、左侧心室收缩末期内径(LVDs)、左侧心房内径(LAD)和左侧心室射血分数(LVEF)。结果患者于术后1 d复查LVDd、LVDs和LAD,均比术前明显缩小,差异有明显统计学意义(P<0.01);于术后1 d复查LVEF,比术前明显减少,差异有统计学意义(P=0.005);于术后3个月时复查,LVEF仍然比术前低,差异有统计学意义(P=0.022);所有患者均未发生症状性心力衰竭。结论国产封堵器治疗PDA安全、有效,成功率高,并发症少,可以改善PDA患者的高动力循环状态。Objective To investigate the effect of patent ductus arteriosus（PDA） domestic occluder therapy on left ventricular volume and systolic function. Methods From March 2014 to October 2016, 40 patients with PDA were collected in Liaocheng second people＇s hospital and all patients with PDA were treated by operation with domestic occluder and delivery system. Left ventricular end diastolic diameter（LVDd） and end-systolic diameter（LVDs）, left atrial diameter（LAD）, left ventricular ejection fraction（LVEF） were measured by echocardiography before operation, 1 day and 3 months postoperatively. Results LVDd, LVDs and LAD were significantly smaller at 1 day after operation than those before operation, the difference was statistically significant（P〈0.01）; LVEF was significantly less at 1 day after operation than that befor operation, the difference was statistically significant（P = 0.005）; LVEF was lower at 3 months after operation than that before operation, the difference was statistically significant（P = 0.022）. All patients did not develop symptomatic heart failure. Conclusions Domestic occluder treatment for patients with PDA is safe and effective. It has a high success rate and fewer complications. Domestic occluder treatment can improve the high power cycle status of patients with PDA.
