滋养脾阴汤治疗胃黏膜异型增生的疗效及对Bcl-2、p53表达的影响

摘　　要：目的:探讨滋养脾阴汤治疗胃黏膜异型增生临床疗效及对Bcl-2、p53表达的影响。方法:将66例胃黏膜异型增生患者随机分为对照组(33例)与治疗组(33例)。对照组予胃复春片治疗,治疗组给予滋养脾阴汤治疗。两组疗程均为24周。比较两组疗效、治疗前后主要症状积分、Bcl-2和p53阳性表达指数及用药期间不良反应发生情况。结果:治疗组总有效率(93.9%)高于对照组(69.7%),差异有统计学意义(P<0.05);与治疗前比较,两组治疗后主要症状积分均明显降低,差异有统计学意义(P<0.05);治疗组治疗后主要症状积分均明显低于对照组(P<0.05);与对照组比较,治疗组患者治疗后的p53呈强阳性表达,颗粒更为粗大且有的呈融合状,Bcl-2阳性表达情况明显偏弱,颗粒密度也明显降低;治疗组治疗后Bcl-2阳性表达指数明显低于对照组,p53阳性表达高于对照组,差异均有统计学意义(P<0.05);两组均未见明显药物不良反应。结论:滋养脾阴汤治疗胃黏膜异型增生具有显著的疗效,可明显降低患者Bcl-2基因阳性表达,提升p53抑癌基因阳性表达。Objective： To investigate the effect of Ziyang Piyin decoction on clinical efficacy and expression of Bcl-2 and p53 in gastric mucosal dysplasia. Methods： The 66 cases of patients with gastric mucosal dysplasia were randoMy divided into control and treatment groups, 33 cases in each. The control group was given Weifuchun, and treatment group was treated with Ziyang Piyin decoction. The treatment course was 24 weeks in two groups. The therapeutic effect, the main symptom scores, Bcl-2 and p53 positive expression index and adverse reactions occurred during the treatment were compared between the two groups. Results： The treatment group showed higher total effeetive rate （93.9%） than the control group （69.7%）, and the difference was statistical （P〈0.05）; The main symptom scores decreased in the two groups, and the difference was statistical （P〈0.05）; The treatment group showed lower symptom scores than the control （P〈0.05）; Compared with the control group, the treatment group showed strongly positive expression of p53, and the particles are more coarse and some were fusion; the Bcl-2 positive expression was weak in treatment group, and the particle density was significantly reduced; After treatment, the treatment group showed lower Bcl-2 positive expression and higher p53 positive expression than eontrol group （P〈0.05）. There was no obvious adverse drug reaction in the two groups. Conclusion： Ziyang Piyin decoction shows significant effect on dysplasia of gastric mucosa; it can significantly reduce the positive expression of Bcl-2 gene and enhance the positive expression of p53 tumor suppressor gene.
