摘 要:目的比较和评估两种乳腺癌筛查方法的应用价值。方法选取贵阳中医学院第二附属医院及贵阳市第一人民医院2012年01月至2015年12月自愿参加乳腺癌筛查并有活检结果的253例病例进行研究。253例乳腺包块住院患者,诊断医师进行针吸穿刺后每例制成2张薄层涂片,1张涂片做巴氏染色,用于常规细胞学检查。另1张经Feulgen染色,用自动细胞DNA定量分析系统检测,每例均有病理结果证实。结果以活检结果作为金标准,253例中共有180例为乳腺良性肿瘤,7例为非典型增生及原位癌,66例为恶性。采用细胞DNA定量分析检测的敏感性和特异性分别为93.15%与95.89%,而常规细胞学检测的敏感性和特异性分别为80.82%与100%。同一个标本同时采用两种方法联合检测,敏感性可提高到94.52%。结论细胞DNA定量分析技术可与常规细胞学联合开展,应用在乳腺癌筛查中。Objective The research aimed to compare and evaluate the application value of two breast cancer screening methods. Methods 253 cases of biopsy from who volunteered for breast cancer screening were selected in Guiyang First People's Hospital and the Second Affiliated Hospital of Guiyang Medical College, from January 2012 to December 2015. After needle aspiration, 2 thin layers of smear were made by diagnostician from 253 patients with breast mass. One smear test was used for routine cytological examination, the other piece was dyeing by Feulgen stai- ning and then analyzed by automatic cell DNA quantitative analysis system. Each patient was confirmed by pathologi- cal results. Results The biopsy results were used as criteria,there were 180 cases of benign breast tumors,7 cases of atypical hyperplasia and carcinoma in situ,and 66 cases of malignant tumors of the 253 patients. Sensitivity and spe- cificity of automatic cell DNA quantitative analysis system were 93.15 % and 95.89%respectively,and the sensitivity and specificity of conventional cytology test were 80.82% and 100% respectively. The two methods were used to analyze the same specimen and the sensitivity was increased to 94.52 %. Conclusion Cell DNA quantitative analysis technology can be combined with conventional cytology in breast cancer screening.
