摘　　要：目的调查重症监护室患者家属手卫生依从性,分析其影响因素,为预防和控制医院感染提供依据。方法对2017年1月-2017年2月ICU患者家属进行手部清洁度ATP荧光检测,隐蔽观察手卫生行为,同时对可能影响手卫生的因素进行问卷调查。利用单因素分析和多因素Logistics回归分析手卫生依从性的影响因素。结果纳入调查的对象共128名,手卫生知识得分率为33.22%,手部清洁度平均168.69 RLU,手卫生合格率15.23%。其中手卫生执行良好的有28人,占比21.88%。分析显示手卫生知识和手卫生目的是影响依从性的独立性因素,对手卫生知识有所了解的调查对象手卫生执行的好(OR=5.81,P<0.05),以"保护自己"为手卫生目的调查对象手卫生行为执行较好(OR=2.71,P<0.05)。结论重症监护室患者家属对手卫生知识掌握差,手卫生依从性亟待提高,应加强监督和管理。Objective To investigate the compliance of the family members of the severe patients in Intensive Care Unit（ ICU） and to analyze the influencing factors for the prevention and control of nosocomial infection. Methods The hand cleanliness of the severe patients＇ family members in ICU from January to February in 2017 were examined by hand ATP cleanup,and their hand hygiene behaviors were observed by a discreet way. Meanwhile,the factors that might affect hand hygiene were investigated by questionnaires,and the factors influencing the compliance of hand hygiene were analyzed by single factor analysis and multiple factor logistics regression. Results 128 subjects were enrolled. Among which the average score of hand hygiene knowledge was 33. 22%. The average hand cleanliness was 168. 69 RLU,and the qualification rate of hand hygiene was 15. 23%. There were 28 people with good hand hygiene,which accounted for 21. 88%. The analysis showed that the hand hygiene knowledge and the purpose of hand hygiene were the two independent factors of hand hygiene compliance. With a well understanding of the hand hygiene knowledge,the hand hygiene compliance was good（ OR = 5. 81,P〈 0. 05）; while with the purpose of ＂In order to protect yourself＂,the hand hygiene compliance could perform better（ OR = 2. 71,P〈 0. 05）. Conclusion The hand hygiene knowledge of the severe patients＇ family members is still poor.The compliance of hand hygiene needs to be improved by knowledge and purpose education,and the hand hygiene management and supervision should be strengthened.
