盆腔器官脱垂患者压力式盆底肌力特征分析

摘　　要：目的应用压力式盆底肌力测试仪分析测试盆腔器官脱垂盆底肌收缩力特点。方法选择2017年6-10月就诊于北京大学人民医院妇科的盆腔器官重度脱垂患者35例作为研究组,选择同期因良性疾病住院的非脱垂患者36例为对照组。以问卷调查形式记录两组患者一般情况,两组均采用Oxford盆底肌力评估法和压力式盆底肌力测试仪测试法进行盆底肌力评估。结果研究组阴道指诊盆底肌力（2.1±0.8）级,显著低于对照组（P〈0.05）。研究组盆底Ⅰ类肌力强度为（11.9±7.7）mm Hg（1 mm Hg=0.133 kPa）,盆底Ⅱ类肌力强度为（18.8±9.2）mm Hg,盆底Ⅰ类肌纤维疲劳度为（-25.6±33.0）%,盆底Ⅱ类肌纤维募集时间为（1.2±1.0）s,与对照组比较,差异均有统计学意义（P〈0.05）。结论盆腔器官脱垂与盆底肌收缩力下降有关;压力式盆底肌力测试盆底肌力与传统阴道指诊结果相符,更精确量化,可用于盆腔器官脱垂的筛查、诊治及随访复查,值得推广。Objective Application of pressure-type pelvic floor muscle strength test instrument analysis of pelvic floor contractility characteristics of pelvic organ prolapse. Methods 35 cases with severe prolapse were the study group. 36 cases with non-prolapsed hospitalized for benign diseases in the same period were the control group.The general information of the patients was recorded by questionnaire. The pelvic floor muscle strength was assessed by Oxford pelvic floor muscle strength test and pressure pelvic floor test. Results The muscle strength of pelvic floor of the study group（2.1±0.8） was significantly lower than that of the control group. In the study group, the muscle strength of pelvic floor Ⅰ was（11.9±7.7） mm Hg, the strength of pelvic floor Ⅱ was（18.8±9.2） mm Hg. The fatigue of class Ⅰ myofibers was（-25.6±33.0）%, and the recruitment of pelvic floor muscle fibers was（1.2±1.0） s. They were compared with the control group, and have significant differences（P〈0.05）. Conclusion Pelvic floor muscle contractility decreased in the patient with prolapse. Pressure pelvic floor muscle test pelvic floor muscle strength consistent with the traditional vaginal fingering results. It is more accurate quantification and can be used for pelvic organ prolapse screening, diagnosis, treatment and follow-up review.
