摘  要:目的探讨常规超声及CEUS在肺周围性病变中的应用价值。方法收集因胸部CT发现存在肺周围性病变的患者120例,对所有患者均行常规超声及CEUS检查。采用常规超声检查明确病灶的部位、大小、形态、回声、血流信号及与毗邻组织的关系;通过CEUS检查观察病灶的增强方式、增强程度、增强均匀度,并进行时间-强度曲线分析,获得相关参数,包括病灶的始增时间(RT)、峰值强度(PI)、平均渡越时间(MTT)、曲线下面积(AUC)、半洗出时间(TPH)、开始廓清时间、下降支斜率(DS),并对良恶性病灶的上述参数进行统计学分析。结果 120例患者中,良性52例,恶性68例。良恶性病变的形态比较差异有统计学意义(P<0.05),良性病变多呈楔形或类三角形,恶性病变多呈类圆形或不规则形;良恶性病变的RT、DS及造影剂开始廓清时间差异有统计学意义(P均<0.001),PI、MTT、AUC、TPH差异无统计学意义(P均>0.05)。结论常规超声和CEUS可为肺周围性病变的诊断提供重要信息。Objective To investigate the value of conventional ultrasound and CEUS in the diagnosis of peripheral lung lesions. Methods Totally 120 patients with peripheral lung lesions found with chest CT were enrolled. Conventional ultrasound and CEUS were performed. The site, size, shape, echo and blood flow signals, as well as the adjacent tissue of lesions were evaluated with conventional ultrasonography. Then, the contrast enhancement pattern, degree and uniformity of lesions were observed with CEUS, and the time-intensity curve analysis was performed to obtain the relevant parameters, including the rise time (RT), peak intensity (PI), mean transit time (MTT), area under the curve (AUC), half washout time (TPH), clearance time and descending slope (DS). The above parameters of benign and malignant lesions were statistically analyzed. Results Among 120 cases, there were 52 benign and 68 malignant tumors. Statistically significant differences were found in morphological comparison between benign and malignant lesions (P〈0.05), while benign lesions were more multi wedge-shaped or triangular shape, and malignant lesions were more round or irregular shape. RT, DS and clearance time of benign and malignant lesions were statistically different (all P〈0.001), but there was no significant difference in PI, MTT, AUC and TPH (all P〉0.05). Conclusion Conventional ultrasound and CEUS can provide important information for diagnosis of peripheral lung lesions.
