活血生肌方外用对糖尿病足患者溃疡创面修复效果及生活质量影响

摘　　要：目的：探讨活血生肌方外用对糖尿病足患者溃疡创面的修复效果及生活质量的影响。方法：收集2013年2月—2015年11月来我院门诊或住院治疗的糖尿病足患者78例,按随机数字表法分对照组和实验组,对照组采用降糖、抗感染、改善微循环、营养神经等对症治疗;实验组在此基础上采用活血生肌方外用,持续治疗2周为1个疗程,共治疗3个疗程。治疗前后记录创面面积、创面氧分压和创面pH值等,对比生活质量SF-36评分,抽取肘静脉血测定血清b FGF、VEGF、PDGF含量,同时对比临床疗效。结果：对照组有效率74.36%低于实验组有效率92.31%（P〈0.05）;与治疗前比,两组治疗后创面面积、创面pH值降低,创面氧分压升高,治疗后生理功能、生理职能、躯体疼痛、总体健康评分升高,治疗后血清b FGF、VEGF、PDGF含量升高（P〈0.05）;与对照组比,实验组治疗后创面面积、创面pH值较低,创面氧分压较高,治疗后生理功能、生理职能、躯体疼痛、总体健康评分较高,治疗后血清b FGF、VEGF、PDGF含量较高（P〈0.05）;两组患者治疗期间未出现任何明显不适,未出现过敏反应而中断治疗。结论：活血生肌方外用有助于提高糖尿病足患者溃疡创面的修复效果,提高生活质量,与上调b FGF、VEGF、PDGF表达有关,是一种有效的治疗方案。Objective：To investigate the effect of Huoxue Shengji Decoction on the healing effect and quality of life in patients with diabetic foot ulcers. Methods：A total of 78 diabetic foot patients in our hospital from February 2013 to November 2015 were collected and randomly divided into control group and experimental group,the control group was treated with hypoglycemic,anti infection,improving microcirculation,nutrition,nerves and other symptomatic treatment,and the experimental group was treated more with Huoxue Shengji Decoction for 2 weeks,and 2 groups were treated for 3 courses. The wound surface area,wound oxygen pressure and wound pH value were recorded before and after treatment. Compared with the life quality SF-36 score,the elbow vein blood was extracted and the serum levels of b FGF,VEGF and PDGF were measured,and the clinical efficacy were compared. Results：The effective rate of the control group was 74.36%,which was lower than the experimental group 92.31%（P〈0.05）. Compared with before treatment,the wound area,wound pH decreased in two groups after treatment,levels of oxygen partial pressure increased,physiological function increased,physiological function,body pain,general health score increased after treatment,serum b FGF,VEGF and PDGF contents increased after treatment（P〈0.05）. Compared with control group,experimental group after treatment,the wound area,wound pH were lower,oxygen pressure wound were high,physiological function,physiological function,body pain,general health score were higher,serum b FGF,VEGF and PDGF contents were higher after treatment（P〈0.05）. There was no obvious discomfort in 2 groups during the treatment period,and there was no allergic reaction and the treatment was interrupted. Conclusion：Application of Huoxue ShengjiDecoction can improve the healing effect of ulcer wounds in diabetic foot patients and improve the quality of life,and is related to the up regulation of the expressions of b FGF,VEGF and PDGF,and it is an effective treatment plan.
