鼾症患者ICU健康服务前移心理护理比较研究

摘　　要：目的：通过针对鼾症患者的术前访视,利用中医理论指导术前患者的心理健康教育,探讨ICU健康服务前移的可行性。方法：选取2016年10月—2017年2月盛京医院第二ICU病房鼾症患者共40例,采用半随机方法,分为心理干预组（实验组）和对照组,每组20例。实验组患者采用ICU健康服务前移,在术前1 d,ICU护士对其进行术前访视,向患者解释相关护理行为,针对患者临床症状表现给予相应的中医护理;对照组的患者在术后转入ICU病房,护士给予常规的护理及宣教。以心理量表SAS评分,因紧张焦虑会直接引发的血压、心率等临床指标改变以及护理期间意外拔管率,综合评价实验组与对照组患者术后康复情况,探讨ICU健康心理护理服务前移的效果。结果：实验组患者对护理满意度升高,非计划性拔管的发生率降低,SAS总分相较于对照组降低,具有统计学意义;血压、心率均值低于对照组,P〈0.05。结论：ICU健康服务前移的心理干预,具有降低患者护理期间紧张焦虑情绪,降低手术风险以及术后并发症,提高护理质量的应用价值和意义;体现了中医心主神明的作用,为临床鼾症患者的术后护理提供了中医理论指导。Objective：To investigate the feasibility of ICU health service by using the theory of traditional Chinese medicine to guide the mental health education of the patients before operation. Methods：40 cases of snoring in second ICU ward of Shengjing hospital from October 2016 to 2017 February were randomly divided into a psychological intervention group（experimental group）and a control group（20 cases each）. The experimental group was treated by the ICU health service forward,in the day before surgery,ICU nurses on the preoperative visit,explaining the related nursing to patients,for patients with clinical manifestation of TCM give corresponding nursing;the patients in the control group were transferred to the ICU after the surgery,the nurse nursing and routine education. Taking the psychological scale SAS score,blood pressure,heart rate and other clinical indicators and nursing care during the accidental extubation rate as the index,to evaluate the experimental group and the control group comprehensive evaluation of postoperative rehabilitation of patients,to explore the forward ICU mental health nursing service effect. Results：The nursing satisfaction increased and unplanned extubation decreased in the experimental group,and the total score of SAS decreased compared with the control group,which was statistically significant. The mean blood pressure and heart rate were lower than that of the control group,P〈0.05. Conclusion：The psychological intervention of ICU health service forward,can reduce the nursing of the patients during the period of anxiety,reduce the risk of surgery and postoperative complications,the application value and significance to improve the quality of nursing;reflects the Chinese heart function of＂heart governs the mental activitiec＂,for clinical snoring patients postoperative nursing provides the guidance of Chinese medicine theory.
