天疱疮患者医院感染的相关因素及感染病原菌耐药性分析

摘　　要：目的探讨天疱疮患者医院感染的相关因素及感染病原菌的耐药性。方法收集医院2011年1月-2016年1月收治的天疱疮患者129例,同时分析天疱疮患者并发感染的相关因素和病原学。结果共25例患者发生感染,感染率为19.38%;使用激素+丙种球蛋白、皮损积分≥4分、病情严重度为重度、白蛋白<30g/L、病程≥6月为患者感染的相关因素(P<0.05);多因素回归分析显示皮损积分≥4分、病情严重度和白蛋白<30g/L是天疱疮患者并发感染的影响因素(P<0.05);金黄色葡萄球菌对青霉素G、庆大霉素和四环素耐药率较高。结论天疱疮患者并发感染的相关因素较多,感染病原菌的耐药性较高,临床应针对相关因素预防感染的发生。OBJECTIVE To explore the related factors for nosocomial infections in patients with pemphigus and analyze the drug resistance of pathogens causing infections.METHODS A total of 129 patients with pemphigus who were treated in hospitals from Jan 2011 to Jan 2016 were enrolled in the study,and the related factors for the infections and etiological characteristics of the pemphigus patients were observed.RESULTS Totally 25 patients had infections,with the infection rate 19.38%.The use of hormones plus gamma globulin,skin lesion score no less than 4 points,severe illness condition,albumin level less than 30 g/L and course of disease no less than 6 months were the related factors for the infections（P0.05）.The multivariate regression analysis showed that the skin lesion score no less than 4 points,severe illness condition and albumin level less than 30 g/L were the influencing factors for the infection in the patients with pemphigus（P0.05）.Staphylococcus aureus strains were highly resistant to penicillin G,gentamicin and tetracycline.CONCLUSIONThere are a variety of related factors for the infection in the patients with pemphigus,and the pathogens are highly drug-resistant.It is necessary for the hospital to prevent the infections according to the related factors.
