同指指背逆行岛状筋膜蒂皮瓣在修复指端软组织缺损中的应用

摘　　要：目的探讨同指指背逆行岛状筋膜蒂皮瓣修复指端软组织缺损的手术方法及疗效。方法 2011年4月-2016年4月,应用同指指背逆行岛状筋膜蒂皮瓣修复指端软组织缺损80例94指。结果 94指皮瓣均成活,61例随访3~18个月,平均10个月,皮瓣颜色、皮肤质地及弹性良好,外形佳,皮瓣两点辨别觉为7~10 mm,皮瓣供区外观满意,手指运动功能良好。结论同指指背逆行岛状筋膜蒂皮瓣修复指端软组织缺损,作为经典术式之一,有手术操作简单、切取方便、不伤及指固有动脉及神经、皮瓣血管蒂较长且旋转弧度大的优势。Objective To investigate the surgical methods and efficacy of the dorsal reverse digital island flap in repairing soft tissue defects of fingertip. Methods From April 2011 to April 2016, 94 fingers of 80 cases with soft tissue defects of fingertip and digital bone exposure were treated with dorsal reverse digital island flap. Results A11 cases survived completely. After a follow-up ranging from 3 to 19 months with an average of 10months, 72 fingers of 61 cases after reconstruction had satisfactory appearance. The two point discrimination of the flap was 7-10 ram. Conclusion Reverse digital island flap is one of the classical methods to repair the defect of fingertip soft tissue with many advantages, such as simple operative procedure, convenient operation, no hurting the interphalangeal artery and nerve, long vascular pedicle, large rotation angle in radians.
