摘　　要：目的通过改良穿支皮瓣手术设计修复老年患者下肢皮肤软组织缺损以解决老年患者创面覆盖的难题。方法 2012年2月-2016年8月,采用改良穿支皮瓣修复老年患者下肢皮肤软组织缺损35例,其中皮肤缺损4.0 cm×3.0 cm^9.0 cm×5.0 cm,病程7~40 d,皮瓣切取范围5.0 cm×4.0 cm^10.0 cm×6.0 cm,供区直接缝合。结果术后35例供受区伤口均Ⅰ期愈合,移植皮瓣均成活。35例随访5~26个月,皮瓣质地良好,成活佳。结论改良穿支皮瓣修复老年下肢创面皮肤软组织缺损,修复后效果佳,能达到修复目的 。Objective To solve the problem of elderly patients with wound coverage, design the modified perforator flap for repairing the elderly patients with skin and soft tissue defect of lower extremity. Methods From February 2012 to August 2016, using a modified perforator flap in treatment of elderly patients with skin and soft tissue defect of lower extremity in 35 cases. Skin defect was from 4.0 cm×9.0 cm to 3.0 cm×5.0 cm, course of disease was 7~40 d. Flap ranged from 3.0 cm×10.0 cm to 5.0 cm×5.0 cm. Donor site was direct sutured. Results The wound of donor and accept in site 35 cases all primany healed, transplantation of skin flap survived. 35 cases were followed up for 5 months to 26 months. The texture of the flap was well and the survival rate was good. Conclusion It can obtain good effect that using modified perforator flap to repair the elderly patients with skin and soft tissue defect of lower extremity.
