某院住院药房退药现状分析与建议

摘　　要：目的调查分析某院住院药房的退药现状,提出相应改进措施以减少和规范退药行为。方法利用医院HIS系统和Excel软件,对随机抽取的该院2015—2016年住院药房387张退药申请单按退药科室、品种、剂型、原因进行回顾性统计分析。结果退药科室主要集中在重症监护病房、心血管内科、消化内科等,退药品种和剂型分别以抗菌药物及注射剂为主,患者原因是退药的主要因素。结论为了有效减少并规范退药行为的发生,医院应加强综合药房管理力度,建立更加完善的退药体制;提高医嘱制订、执行的质量;完善药学服务、改善服务流程;并加强医、护、药、患之间的相互沟通,从而保证患者用药的安全、合理、有效。Objective To investigate and analyze the present situation of drug withdrawal in Inpatient dispensary of hospital,and put forward the corresponding improvement measures to reduce and standardize the drug withdrawal behavior.Methods Using the hospital HIS system and Excel software,According to the drug withdrawal department, varieties,dosage form and reason.A retrospective statistical analysis was made on the randomly selected 387 withdrawal drug application forms in Inpatient dispensary of our hospital in 2015—2016.Results The drug withdrawal section is focused on ICU,cardiac medicine department,digestive department and so on.The drug withdrawal varieties and dosage forms were mainly based on antimicrobial agents and injections,Causes of the patients are the major factors of drug withdrawal.Conclusion In order to effectively reduce and standardize the drug withdrawal behavior, Hospital should strengthen the comprehensive pharmacy management,establish a more perfect drug withdrawal system, improve pharmaceutical services and service processes,strengthen communication between doctors,nurses,pharmacists and patients,so as to ensure the safety, reasonable and effective use of drugs.
