摘 要:目的了解东城区孕妇中孕期血清学产前筛查结果,探讨产前筛查结果与21-三体综合征、18-三体综合征及开放性神经管畸形三种出生缺陷之间的关系,为临床诊断与治疗提供理论依据。方法东城区产前筛查机构采集孕妇(15-20^(+6))空腹静脉血,结合孕周、空腹体重、年龄等因素进行检测,登记检测结果,按季度上报筛查结果并及时追访妊娠结局。结果 2013-2016年东城区中孕期产前筛查率呈现上升趋势;筛查高风险率也出现上升趋势,21-三体综合征高风险检出率最高,其次为神经管畸形、18-三体综合征;经过产前诊断确诊,筛查高风险组的21-三体综合征、18-三体综合征、神经管畸形及其他畸形的发生率高于筛查低风险组。结论产前筛查作为一种无创、经济和简便的筛查方法,能够有效地筛查出正常孕妇人群中的高危人群,检测出严重出生缺陷21-三体综合征、18-三体综合征、神经管畸形等疾病的风险值,这对预防出生缺陷具有一定临床应用价值。Objective:To learn serum prenatal screening results of pregnant woman in second trimester in Dongcheng district and discuss the relationship between the serum prenatal screening results and the three birth defects of 21-trisomic syndrome,18-trisomic syndrome and Neural tube defects,providing theory basis for clinical diagnosis and treatment. Methods:The fasting venous blood of pregnant women(15-20^+6)were collected and tested by prenatal screening agencies in Dongcheng district,combined with gestational age,fasting weight,age and other factors. The prenatal screening results were Registered,and reported quarterly. The pregnancy outcomes were followed in time. Results:The prenatal screening rates and the screening risk rates in second trimester were increased in Dongcheng district from 2013 to 2016. The screening risk rate of 21-trisomic syndrome was highest,followed by Neural tube defect and 18-trisomic syndrome. After prenatal diagnosis,the incidence of 21-trisomic syndrome,18-trisomic syndrome,Neural tube defect and other birth defects in high-risk group were higher than that in the low-risk group. Conclusion:As a noninvasive,economical and sample screening method,the serum prenatal screening can effectively screen the high-risk groups from normal pregnant women and detect the risk of severe birth defects,such as 21-trisomic syndrome,18-trisomic syndrome and Neural tube defect. It is certain clinical application value for the birth defects prevention.
