摘 要:探讨人鼻病毒(Human rhinovirus,HRV)在河北省部分地区发热呼吸道症候群(Febrile respiratory syndrome,FRS)哨点监测病例中的感染情况。2013~2015年,从河北省部分地区发热呼吸道症候群哨点监测病例中采集标本792份,用多重实时荧光RT-PCR方法对9种常见的呼吸道病毒病原体进行筛查,HRV阳性标本再用RT-PCR方法扩增VP4/VP2靶基因片段,进行序列测定和分析。病原构成比位于前三位的病原分别是人副流感病毒(Human parainfluenza virus,HPIV),HRV和人流行性感冒病毒(Human influenza virus,Flu)。HRV阳性标本73份,检出率为9.22%。其中HRV混合其他呼吸道病毒感染的有12份(12/73,16.44%),与HPIV的混合感染率最高。本研究发现,HRV感染以5岁以下儿童为主,占全部HRV感染病例的60.27%;没有明显的季节分布趋势;住院病例比门急诊病例检出率高;下呼吸道感染患者中HRV的检出率高于上呼吸道感染患者;HRV在不同性别病例中检出率差别没有统计学意义。本研究共检测到HRV的20个血清型,其中A组HRV包括11个血清型,C组HRV包括9个血清型,B组HRV未检出。本研究显示,河北省存在HRV多个血清型的共循环,亟需加强河北省发热呼吸道症候群中HRV的监测,为HRV防控策略制订提供基础数据。We wished to investigate human rhinovirus(HRV)infection in patients with febrile respiratory syndrome(FRS)in sentinel surveillance hospitals in Hebei Province,China,from 2013 to 2015 and 792 samples were collected from these patients.Multiplex real-time reverse transcription-polymerase chain reaction(RT-PCR)was undertaken to detect nine common respiratory viral pathogens.For HRV-positive samples,VP4/VP2 gene fragments were amplified by RT-PCR.VP4/VP2 regions were sequenced and analyzed.The human parainfluenza virus(HPIV)was the pathogen detected most frequently,followed by the HRV and human influenza virus.A total of 73 specimens were positive for HRV(9.22%).12 samples were HRV mixed with other respiratory viruses(12/73,16.44%),and the most frequently co-detected virus was the HPIV.Children aged 5 years had the highest prevalence of infection(60.27%).There was no clear seasonal pattern for HRV infection.The prevalence of detection for hospitalized cases was higher than that for patients in outpatient and emergency departments.The prevalence of HRV detection in the lower respiratory tract was higher than that in the upper respiratory tract.The prevalence of HRV detection between males and females was not significantly different.Twenty serotypes were detected:11 serotypes of HRV-A and 9 serotypes of HRV-C.HRV-B was not detected.Our study shows that there were several serotypes of HRV co-circulating in Hebei Province,China.These results provide basic information for a formulation strategy for the prevention and control of HRV infection in patients with FRS in Hebei Province,China.
