彩色多普勒超声评价左肾静脉受压与左精索静脉曲张的关系

摘　　要：目的 :探讨左肾静脉受压与左侧精索静脉曲张的关系。方法 :选择经彩色多普勒超声 (CDFI)确诊的左侧精索静脉曲张患者 6 4例 ,以 5 8例正常人为对照 ,仰卧位超声测量脊柱前、左侧扩张部肾静脉内径和肠系膜上动脉 (SMA)和腹主动脉 (AAO)之间夹角。结果 :6 2例患者和 5 5例正常人检查成功。与对照组相比 ,左侧精索静脉曲张患者脊柱前肾静脉变窄、脊柱左侧肾静脉内径增宽、左肾静脉扩张部内径 /狭窄部内径比例增大 ,SMA与AAO夹角变小。结论 :左肾静脉受压是左侧精索静脉曲张的主要原因之一。 CDFI诊断左肾静脉压迫综合征简单实用、成功率高 ,值得推广。Objective:To discuss the relationship of left renal vein entrapment syndrome (LRVES)and left varicocele.Methods:The left renal veins(LRV) of 58 normal subjects and 64 patients with left varicocele diagnosed by color Doppler flow imaging(CDFI)were displayed by two dimensional and color Doppler ultrasonography.Results:Sixty two LRV of 64 patients and 55 LRV of 58 normal subjects were successfully displayed.In the left varicocele patients,the diameter of LRV anterior to ventebral column(VC)was slender(stenotic portion,DS entraped),while the diameter of LRV left to the VC was widened.(Dw).Dw was twice more than Ds in 35 cases,and LRVES was diagnosed.In the normal group,as compared with the 35 cases(35/62,56%),only 3 patients(3/55,5%)were diagnosed as LRVES, P <0 01.Conclusions:Left renal vein entrapment is one of the most important factors in left varicocele.CDFI is a simple and practical way to evaluate left renal vein entrapment syndrome.
