“火郁发之”治疫治法举隅

摘　　要：火郁是疫病的一个重要病机,古代医家常以"火郁发之"治之。温邪所致热疫、温疫,火热炽盛之中也有火郁的病机,治以清热解毒兼透散,"火郁发之"贯穿治疗全程。以表里传变、三焦传变为特点的温疫、湿热疫,或因火热太盛而致火郁,或因湿遏而致火郁,治疗以逐秽解毒、因势利导为总则,行表里分治、三焦分治,以"火郁发之"治在表、半表半里、上焦的阶段,发邪致表,透邪外出。寒疫、湿疫,因寒湿阻遏而致火郁,"火郁发之"是在祛除寒湿的同时清热散火。正虚患疫或疫病后期伤正,火郁不得发,以扶正散热升清之法来"火郁发之"。Depressing of fire is an important pathogenesis in epidemic disease, and is usually treated with the theory of ＇dispersing fire for treating depressing of fire＇ （Huo Yu Fa Zhi） by Chinese ancient doctors. According to pathogenic evils and disease progress, there are 4 kinds of therapeutic method. Some epidemic disease caused by warm evils, in which pathogenesis of depressing of fire could be found in the crowd of heat syndromes, could be treated with the method of heat-clearing and detoxifying and simultaneously heat-diffusing under the theory of Huo Yu Fa Zhi throughout the treatment. Some epidemic disease caused by warm evils or damp-heat, having the disease progress through exterior and interior, or sanjiao, in which pathogenesis of depressing of fire could be caused by excessive fire or obstruction of damp, should be treated with a principle of taking advantage of situation to drive the pathogenic evils and pathology products away, and Huo Yu Fa Zhi would be used in the stage of exterior syndrome, half exterior and half interior syndrome, or shangjiao syndrome, taking the method of expelling pathogenic factors. Some epidemic disease caused by pathogenic cold or damp, in which pathogenesis of depressing of fire could be caused by obstruction of cold or damp, could be treated with the therapeutic method of dispelling cold and dampness and simultaneously clearing and diffusing fire under the principle of Huo Yu Fa Zhi. Some frail patients with epidemic disease, whose healthy energy could not dispel depressed fire out, therefore should be treated with the method of strengthening body resistance to diffuse fire under the principle of Huo Yu Fa Zhi.
