化痰开窍法治疗癫狂的探析

摘　　要：化痰开窍法,泛指通过祛除痰浊治疗痰迷心窍病证的方法。痰是癫狂的共同致病因素,"痰迷心窍"是癫狂的关键病机,"化痰开窍"是癫狂的主要治法。根据导致癫狂"痰迷心窍"的具体证候病机,古今医案医论中论及相应多种辨证论治之法。文章仅就癫病之痰气互结、格塞心窍证,气郁化热、痰火扰心证,脾胃虚寒、痰流心窍证;狂病之心肝火盛、痰火上扰证,心虚有热、痰气内侵,疾瘀互结、蒙蔽心窍,冲气挟痰、上逆乱心证及其相应治法,加以简要论述。此外,还论及癫与狂"痰火证"的区别与转化,为临床辨证论治提供参考。Phlegm for resuscitation method generally dispels turbid phlegm to cure ＇phlegm confuses the mind＇ diseases and syndromes. Sputum is the common pathogenic factor for epilepsy and mania, and ＇phlegm confuses the mind＇ is the key pathogenesis of epilepsy and mania. Hence, ＇phlegm for resuscitation＇ is a major therapeutic method to treat epilepsy and mania. With respect to specific syndrome pathogenesis of ＇phlegm confuses the mind＇ of epilepsy and mania, there have been a variety of syndrome differentiation and treatment methods studied in medical cases and medical theories throughout the history. This paper briefly discussed the syndromes of epilepsy （stagnation of qi and phlegm, gesse heart syndrome; qi depressive heart, phlegm fire attacking the heart, deficiency cold in spleen and stomach and phlegm confuses the mind） and mania （excessive heart-liver fire, phlegm-fire up-disturbing, heart deficiency raw thermal, sputum qi approaching, phlegm accumulating with stagnation, confusion of mind, impact gas pinch sputum and superinverse revolt heart） with relevant therapeutic methods. In addition, it also made a discussion on the differentiation and transformation of sputum-fire syndromes of epilepsy and mania, providing references for clinical syndrome differentiation and treatment.
