解郁活血方治疗冠心病伴焦虑症的作用机理及临床效果

摘　　要：目的：探讨解郁活血方对冠心病伴焦虑症的治疗效果。方法：选取2016年3月—2016年12月在医院接受治疗的冠心病伴焦虑症患者为研究对象,根据随机数字表法将其分为对照组和中药组,对照组给予常规西药治疗,观察组在此基础上给予解郁活血方治疗,两组患者均接受12周的治疗。观察两组患者治疗前后心绞痛、心电图情况,比较两组患者治疗前后HAMA和SAS评分、血浆NO、内皮素和FMD水平的差异。结果：治疗后,中药组心绞痛改善率为95.45%,明显高于对照组（χ2值=4.247,P=0.039）;治疗后,中药组心电图改善率为100.00%,明显高于对照组（χ2值=6.947,P=0.008）;两组患者治疗前HAMA和SAS评分无差别,治疗后,中药组患者HAMA和SAS评分低于对照组（t=11.964、14.221,P〈0.001）;两组患者治疗前血浆NO、内皮素和FMD水平无差别,治疗后,中药组患者的内皮素水平低于对照组,NO和FMD水平高于对照组;两组患者在治疗期间未发生血常规、粪、尿常规异常以及明显肝肾功能损伤等不良反应。结论：解郁活血方对冠心病伴焦虑症有较好的治疗效果,可明显改善患者的临床症状,改善内皮功能。Objective： To investigate the therapeutic effect of Jieyu Huoxue Decoction on coronary heart disease with anxiety. Methods： Patients with coronary heart disease（ CHD） with anxiety disorder treated in our hospital from March2016 to December 2016 were selected as the research object. They were randomly divided into control group and Chinese medicine group according to random number table method. The control group was given conventional western medicine treatment and the observation group was given Jieyu Huoxue Prescription on the basis of the treatment. Two groups were treated for 12 weeks. We observed two groups＇ angina pectoris and ECG before and after treatment and compared the two groups＇ HAMA and SAS score,plasma NO,endothelin and FMD levels. Results： After treatment,the Chinese medicine group＇s angina improvement rate was 95. 45%,significantly higher than the control group＇s（ x^2= 4. 247,P = 0. 039）.In the Chinese medicine group after treatment,ECG improvement rate was 100%,significantly higher than the control group＇s（ χ~2= 6. 947,P = 0. 008）. There was no difference between the two groups＇ HAMA and SAS score before treatment. After treatment,the scores of HAMA and SAS in the traditional Chinese medicine group were lower than those in the control group（ t = 11. 964,14. 221,P 〈 0. 001）. The levels of plasma NO,endothelin and FMD were not different between the two groups before treatment. After treatment,the level of endothelin in patients of TCM group was lower than that of control group and the levels of NO and FMD were higher than those of control group. There were no adverse reactions of blood routine,stool and urine routine,or liver and kidney function damage in the two groups. Conclusion： Jieyu Huoxue Prescription has a good therapeutic effect on coronary heart disease with anxiety and can significantly improve the clinical symptoms and improve endothelial function.
