水疗配合催眠镇痛对产程及分娩结果的作用研究

摘　　要：目的探讨水疗配合催眠镇痛对产程及分娩结果的作用。方法选取2015年1月至2016年12月本院产科诊治的400例产妇作为研究对象,按随机数字表法分为观察组和对照组各200例,对照组采取常规助产方法;观察组在对照组护理的基础上进行水疗配合镇静催眠疗法;比较两组产妇的疼痛、焦虑、产程时间、产后2h出血量、催产素静脉滴注使用率、自然分娩率及分娩控制感等情况;并记录新生儿1min时的Apgar评分。结果与对照组比较,观察组产妇的各产程及总产程时间,疼痛评分、焦虑评分、产后2h出血量、催产素静脉滴注使用率显著降低,自然分娩率、分娩控制感和满意度显著提高,差异均有统计学意义(P<0.05);两组产妇新生儿1min时Apgar评分差异无统计学意义(P>0.05)。结论水疗配合镇静催眠疗法能够显著降低产妇的疼痛感和焦虑感,减少产程时间并减少产后出血量和催产素的使用率,效果显著,值得在临床推广应用。Objective To explore the effect of hydrotherapy combined with hypnotic analgesia in labor and its effect on delivery outcome.Methods A total of 400 cases of maternal in our hospital from January 2015 to December 2016 were selected as a research objects.According to the random number table method,they were divided into observation group and control group.The control group received routine method of midwifery,while the observation group received the care of spa and the sedative hypnosis.The pain,anxiety,labor time,2 hpostpartum hemorrhage,usage rate of intravenous infusion of oxytocin,rate of natural childbirth,control of childbirth and Apgar score at 1 min for newborns were recorded.Results Compared with the control group,the maternity and total labor time,pain score,anxiety score,2 hpostpartum hemorrhage,and usage rate of intravenous infusion of oxytocin were significantly decreased.Natural delivery rate,childbirth control and satisfaction were significantly improved.The difference was statistically significant（P〈0.05）.There was no significant difference in Apgar scores between the two groups at 1 min（P 〉0.05）.Conclusion The combination of sedation and hypnosis can significantly reduce maternal pain and anxiety,the time of labor and the amount of postpartum hemorrhage and usage rate of oxytocin utilization,the effect is remarkable,it is worth of promotion.
