醒脑开窍针法联合活血解郁汤加减对脑卒中后抑郁症患者HAMD评分及生活质量的影响

摘　　要：目的:探讨醒脑开窍针法联合活血解郁汤加减对脑卒中后抑郁症患者汉密尔顿抑郁量表(HAMD)评分及生活质量的影响。方法选取符合标准的75例脑卒中后抑郁症患者,按照随机数字表法分组,对照组37例给予阿米替林治疗,观察组38例于对照组基础上给予醒脑开窍针法联合活血解郁汤加减治疗,观察比较2组治疗前后HAMD评分变化情况及抑郁疗效,并统计2组毒副反应发生情况及生活质量(QOL)评分。结果:治疗1个月后观察组HAMD评分低于对照组,QOL评分高于对照组,差异有统计学意义(P<0.05);观察组抑郁治疗总有效率为92.11%(35/38),高于对照组72.97%(27/37),差异有统计学意义(P<0.05);观察组毒副反应发生率为15.79%(6/38),对照组为10.81%(4/37),组间对比差异无统计学意义(P>0.05)。结论:醒脑开窍针法与活血解郁汤加减针药联合使用可改善脑卒中后抑郁症患者抑郁症状及生活质量,疗效显著,安全性高。Objective： To investigate the effect of Xingnao Kaiqiao acupuncture combined with Huoxue Jieyu decoc- tion on the scores of Hamilton Depression scale （HAMD） and quality of life in patients with post-stroke depression. Methods： A total of 75 post-stroke depression patients meeting the criteria were selected and grouped according to the random number table method. In the control group, 37 patients were treated with amitriptyline. 38 cases in the observa- tion group were given Xingnao Kaiqiao acupuncture combined with Huoxue Jieyu decoction on the basis of the control group. Observe and compare the changes of HAMD scores before and after treatment in the two groups and the effect of depression, and count the incidence of adverse reactions and quality of life （QOL） scores in the two groups. Results： After one month of treatment, the HAMD score of the observation group was lower than that of the control group, and the QOL score was higher than that of the control group. The difference was statistically significant （P〈0.05）. The total effective rate of depression treatment in observation group was 92.11% （35/38）, which was higher than 72.97% （27/37） in control group. The difference was statistically significant （P〈0.05）. The incidence of toxic side effects was 15.79% （6/38） in the observation group and 10.81% （4/37） in the control group. There was no significant difference between the two groups （P〉0.05）, Conclusion： The combination of Xingnao Kaiqiao acupuncture and Huoxue Jieyu decoction plus or minus acupuncture can improve depressive symptoms and quality of life in patients with post-stroke depression, with significant curative effect and high safety.
