产前超声诊断胎儿骶尾部畸胎瘤的临床价值

摘　　要：目的 探讨产前超声对胎儿骶尾部畸胎瘤的诊断价值.方法 回顾性分析经产前超声检查并经引产后尸检、随访或出生后手术病理证实的骶尾部畸胎瘤12例,分析病变的声像图特征.结果 产前超声诊断的12例胎儿骶尾部畸胎瘤超声表现为骶尾部大小不等的包块,其中囊性6例,囊实性6例;Ⅰ型6例,Ⅱ型4例,Ⅲ型1例,Ⅳ型1例;6例内部未见明显血流,5例可见少量血流,1例血供丰富,形成动静脉瘘;10例为骶尾部畸胎瘤单发畸形,2例合并有其他结构异常.结论 胎儿骶尾部畸胎瘤超声表现多样,产前超声能明确病变的位置、成分、大小、血供及其与毗邻器官的关系,并能识别是否存在其他合并畸形,为评估胎儿预后及早期临床干预提供可靠的依据.Objective To investigate the value of prenatal ultrasound in the diagnosis of fetal sacrococcygeal teratoma.Methods Twelve cases of fetal sacrococcygeal teratoma diagnosed by the prenatal ultrasound and confirmed by autopsy after odinopoeia,follow-up or surgical pathology after delivery were retrospectively analyzed.The ultrasonographic features of the lesion were analyzed.Results Ultrasonography of twelve cases of fetal sacrococcygeal teratoma diagnosed by the prenatal ultrasound presented sacrococcygeal mass with different sizes,of which 6 cases were cystic and 6 cases were cystic solid;6 cases were type Ⅰ,4 cases were type Ⅱ,1 case was type Ⅲ,1 case was type Ⅳ;6 cases had no obvious blood flow in teratomas,5 cases had a small amount of blood flow,1 case had rich blood supply,which formulated arteriovenous fistula;10 cases were single sacrococcygeal teratoma,2 cases were complicated by other structural abnormalities.Conclusions The performance of fetal sacrococcygeal teratoma ultrasonography is various,and prenatal ultrasound can determine the lesion location,composition,size,blood supply and its relationship with adjacent organs,and can identify whether there are other combined malformations,and provide a reliable basis for the evaluation of fetal prognosis and clinical intervention.
