摘　　要：目的 探究自体输血和异体输血对卵巢癌患者免疫功能相关细胞比例的影响.方法 选取卵巢癌确诊患者61例,根据随机数字表法分为自体输血组和异体输血组,输血量均为2～4U.采用流式细胞术检测患者术前1d及术后1、7d外周血中CD3＋T、CD4＋T、CD8＋T、自然杀伤（NK）细胞和Treg细胞比例,分析不同输血方式患者围术期以上细胞的变化.结果 两组患者术前1 d CD3＋T、CD4＋T、CD8＋T、NK和Treg细胞比例比较差异均未见统计学意义（P均＞0.05）.自体输血组术后1 d CD4＋T、CD4＋/CD8＋及NK细胞比例较术前均显著下降（P均＜0.05）,CD8＋T细胞和Treg则显著上调,而在术后7d均恢复至接近术前水平.异体输血组术后1 d CD3＋T、CD4＋T、CD4＋/CD8＋及NK细胞比例均显著下降,而CD8＋T和Treg显著上调,术后7d以上指标与术前比较差异有统计学意义（P＜0.05）.结论 与自体输血比较,异体输血对卵巢癌患者免疫功能影响较大且维持时间持久.Objective To investigate the influence of autologous blood transfusion and allogeneic blood transfusion on immune function in patients with ovarian cancer.Methods Sixty-one patients with ovarian cancer were selected and divided into autologous blood transfusion group and allogeneic blood transfusion group according to random number table method.The blood transfusion volume for each patient was 2-4 U.Ratios of CD3 ＋ T,CD4 ＋ T,CD8 ＋ T,natural killer （NK） cell and Treg cells in peripheral blood were mesured by flow cytometry at 1 d before surgery,1 and 7 d after surgery.The changes of these cells during the perioperative period were analyzed.Results There was no significant difference in the proportion of these cells in the two groups at 1 d before operation （all P 〉 0.05）.For autologous blood transfusion group,ratios of CD4 ＋ T,CD4 ＋/CD8 ＋ and NK cells at 1 d after operation significantly decreased compared with those before operation,and CD8 ＋ T and Treg cells were significantly upregulated （all P 〈 0.05）,and at 7 d after operation they were almostly recovered to the preoperative levels.For allogeneic blood transfusion group,ratios of CD3 ＋ T CD4 ＋ T,CD4 ＋/CD8 ＋ and NK cells all significantly decreased,while CD8 ＋ T and Treg cells significantly increased at 1 d after operation,and there were also significant differences in ratios of these indexes at 7 d after operation compared with those at 1 d before operation （P 〈 0.05）.Conclusions Compared with autologous blood transfusion,allogeneic blood transfusion has greater influence on immune function of ovarian cancer patients,and it also has a long maintenance time.
