摘　　要：目的 对比分析大剂量输血患者采用去白细胞输血和常规输血的临床效果.方法 350例需大剂量输血患者根据输血方式分成对照组（175例,常规输血）和研究组（175例,去白细胞输血）.对比观察组间免疫功能及不良反应情况.结果 研究组输血前后细胞免疫和体液免疫指标比较差异未见统计学意义（P＞0.05）;对照组输血后CD4＋及CD4＋/CD8＋明显下降,IgA、IgE、IgG及IgM均显著下降,且与研究组比较差异有统计学意义（P＜0.05）.研究组不良反应发生率（4.57％）低于对照组（18.86％）,组间比较差异有统计学意义（P＜0.05）.结论 需大剂量输血患者使用去白细胞输血后的不良反应相对较少,且不会对其免疫功能造成严重不良影响.Objective To compare and analyze the effect of leukocyte-depleted transfusion and routine transfusion on patients with massive blood transfusion.Methods A total of 350 patients who needed massive blood transfusion were divided into control group （175 cases,routine blood transfusion）and study group （175 cases,leukocyte-depleted blood transfusion） according to the ways of blood transfusion.The immune function and adverse reactions between the two group were observed and compared.Results There was no significant difference in cellular immunity and humoral immunity indexes of study group before and after tansfusion （P 〉 0.05）;in control group,CD4 ＋ and CD4 ＋/CD8 ＋ decreased significantly after tansfusion,and IgA,IgE,IgG and IgM all significantly decreased,the differences in the above indexes between study group and control group were significant （P 〈 0.05）.Incidence of adverse reaction in study group （4.57%） was lower than that in control group （18.86%）,and the difference was significant （P 〈 0.05）.Conclusions Leukocyte-depleted transfusion has fewer adverse reactions for patients who need massive blood transfusion,and has no severe adverse effect on the immune function.
