老年髋部骨折术后日常活动能力影响因素分析

摘　　要：目的研究影响老年髋部骨折患者术后日常生活能力的相关因素。方法对2010年1月至2015年6月间收治的165例老年髋部骨折手术病例进行随访研究，通过回归分析研究患者的日常生活能力及其相关影响因素。结果Logistic回归分析显示：年龄、手术方式、伤前自主活动能力、术前内科合并疾病、血红蛋白和白蛋白是随访终末日常生活能力的影响因素；性别、骨折类型、ASA分级、伤后至手术间隔时间、麻醉类型、手术时长、术中失血量及住院天数对随访终末日常生活能力无影响。结论临床中应对高龄、伤前ADL能力差、术前内科合并症种类多、血红蛋白和白蛋白水平低的患者重点关注，围手术期加强干预，选择合适的手术方式，采取有针对性、综合的治疗和康复方法以提高患者术后的生活自理能力。Objective To study the impact of related factors on the postoperative basic activities of daily living of elderly patients with hip fracture. Methods A follow-up studies of 165 cases of elderly hip fracture patients with complete follow-up were conducted from January 2010 to June 2015 and the postoperative basic activities of daily living of patients and related factors with regression analysis were researched. Results Logistic regression analysis showed： age, surgical approach, the independent activity before the injury, the type of merger medical illness, hemoglobin and albumin were factors of postoperative basic activities of daily living. Gender, fracture type, ASA classElcation, surgical time from injury, anesthesia, operative time, blood loss and postoperative hospital stay had no effect on the postoperative basic activities of daily living of patients. Conclusions In clinical treatment, the patients who are aged, with low scores of the independent activity before the injury, and more merger medical illness, and low hemoglobin and albumin, should be paid more attention during the perioperation. Of course, appropriate surgical approach should be chosen, a targeted therapy and rehabilitation methods should be taken to improve the postoperative basic activities of daily living of geriatric patients.
