摘  要:目的:探讨高尿酸血症对年龄≤35岁的青年非吸烟人群冠心病发生的影响。方法:回顾性分析2005年1月至2015年12月,因胸痛在北京安贞医院入院,并行冠状动脉造影检查的18~35岁青年非吸烟患者393例。将患者分为冠心病与非冠心病组,对两组中高尿酸血症以及其他冠心病易患因素分布进行对比研究。采用Logistic回归模型分析高尿酸血症与冠心病的关系。结果:青年非吸烟人群中,冠心病患者共246例,非冠心病患者共147例。高尿酸血症在冠心病组中比例明显升高(33.5%vs.19.4%,P<0.01)。血尿酸水平在冠心病组明显高于非冠心病组[(376.04±91.63)vs.(354.62±82.71)μmol/L,P=0.02]。Logistic分析显示,高尿酸血症为冠心病发生的独立相关危险因素(OR=1.846,95%CI=1.086~3.140,P=0.024)。结论:高尿酸血症是35岁以下青年非吸烟人群冠心病的独立相关危险因素。Objective: To explore the correlation between hyperuricemia and coronary artery disease( CAD) in young non-smokers( ≤35 years old). Methods: A total of 393 young adults( aged 18-35 years old) who never smoke undergoing coronary angiography for the first time at our institution from January 1,2005,through December 31,2015 were retrospectively analyzed and were divided into two groups according to the result of coronary angiography,246 in coronary artery disease group and 147 in control group. Logistic regression analysis was adopted to evaluate the relationship between hyperuricemia and CAD. Results: 393 participants were included in this study. The hyperuricemia was present in 28. 0% of participants. Hyperuricemia was significantly higher in CAD group( 33. 5% vs. 19. 4%,P〈0. 01). The CAD patients had significantly higher SUA levels than those without CAD( 376. 04 ± 91. 63) vs.( 354. 62 ± 82. 71) μmol/L( P = 0. 02). Logistic multivariate regression analysis showed that hyperuricemia was an independent risk factor for the presence of CAD( OR = 1. 846,95% CI = 1. 086-3. 140; P = 0. 024). Conclusion: In our essay,we found that hyperuricemia was the independent related risk factor for coronary artery disease in non-smokers aged 18-35 years old.
