颈椎横突孔与椎动脉供血不足影像学分析研究

摘　　要：椎动脉供血不足的原因大致为颈椎因素、椎动脉原有因素等,包括如动脉硬化性狭窄及血管先天畸形,并涵盖了椎动脉发育纤细、走向异常以及神经性因素等。本文详细介绍了颈椎横突孔与椎动脉供血不足之间关系的影像学表现,尤其是对于正常横突孔影像及椎动脉走行、横突孔位置改变、横突孔狭窄以及横突孔周围结构变化导致的椎动脉供血不足进行详细分析,并用实例说明横突孔原发、继发性狭窄,或者是由于横突孔周围结构退变所导致椎动脉扭曲、受压变窄以及交感神经兴奋,甚至是反射性引起椎-基底动脉痉挛均可以致使椎动脉供血不足。由此提出任何导致横突孔狭窄、错位的因素都可造成椎动脉供血不足。The causes of vertebral artery insufficiency are the cervical vertebral factors,the original factors of the cervical vertebral artery,and so on,which included atherosclerotic stenosis and congenital malformations of the cervical vertebral artery,and the fine,abnormal trend and neurogenic factors of vertebral artery development. This paper introduces in detail the relationship between cervical transverse foramen and vertebral artery insufficiency of the imaging findings,especially to analyze the imaging findings of normal transverse foramen and vertebral artery insufficiency caused by vertebral artery walking,change of transverse foramen position,transverse foramen stenosis,and changes of the structure around the transverse foramen. And to illustrate the causes of vertebral artery insufficiency by examples,such as primary and secondary stenosis of transverse foramen,or vertebral artery distortion,compression narrowing,and sympathetic nerve excitability caused by structure surrounding degeneration of transverse foramen,even the reflexive vertebrobasilar spasm. It is suggested that all the factors causing stenosis and dislocation of the transverse foramen can cause vertebral artery insufficiency.
